No Prescreen - Will interview with Manager

  • beds/rooms: 20 provider pods

  • Common diagnosis / Types of patients: Primary Care - newborns to geriatric - skin biospy, Circumsision, wart removal, occassional stitches, splinting

  • Experience or 'Must have' skills: Clinic experience preferred

  • Is there a Manager/Supervisor on each shift?: Yes - RN Lead

  • Do you use Medical Assistants or CNA's?: CNA's & CMA's

  • EMR: AEHR and Meditech

  • Shifts & Scheduling: Manager Scheduling - mixed some 8's and 10s - 0800-1700 or 0700-1800, Manager will create schedule

  • Weekend Requirement: None

  • Holiday Requirement: None

  • Floating Requirements: will float to different primary care departments- Peds, Family, OB, walk-in, Internal Med

  • How many shifts of unit/department specific orientation?: 1 week

    Radius Rule: 50 miles AND candidates may not be full time, part time, or PRN with any facility associated with CommonSpirit, including CHI and/or Dignity.

    RTO needed upfront (7 or less days per 13 week contract). Additional RTO will not be approved, including after submissions.

Under the supervision of a Registered Nurse, the Licensed Practical/Vocational Nurse works in a variety of remote or clinic settings to serve a specific population's health and/or wellness needs. These variety of sites include but are not limited to community or employment immunization clinics, wellness clinics, job safety and workers compensation sites and travel health clinics from pediatric to geriatric age populations.

The Licensed Practical/Vocational Nurse demonstrates the ability to make clinical judgments in an effective and efficient manner under the supervision of an RN or Physician. The Licensed Practical/Vocational Nurse demonstrates critical thinking and performance ability in the coordination of patient care. Consistently performs according to nursing standards and is accountable in providing safe, quality patient care.

Essential Duties and Responsibilities:

  • Under the direct supervision of a Registered Nurse utilizes the nursing process to review plan, implement and evaluate patient care

  • Evaluates a patient’s health status, estimates the level of health risk and informs/provides s feedback to patients to motivate behavior change to reduce health risks

  • Performs health screenings and makes appropriate referrals when indicated

  • Performs interventions according to identified priorities, plan of care, and the Clinic policies and patient care outcome standards

  • Provides a safe, comfortable and therapeutic patient environment

  • Initiates and evaluates patient and/or family education when appropriate

  • Documents patient’s response to nursing and other interventions

  • Monitors the safety and effectiveness of the environment and equipment

  • Knowledge of universal precautions

  • Knowledge of protocols for adverse reactions, reporting of incidents and injuries

  • Knowledge of Workers Compensation assessments and reporting if applicable

  • Knowledge of Immunization and Vaccine schedules and safety

  • Administers vaccines or any necessary medications per protocols

  • Handles storage and transportation of necessary supplies including vaccines

  • Appropriately handles and disposes of biological waste

  • Helps stock and maintain the cleanliness of the site/location where they are stationed

  • Helps set up and dismantle clinic sites in various settings

  • Maintains confidentiality at all times

Minimum Requirements:

  • Current Licensed Practical/Vocational Nurse License in state where they are practicing

  • One year experience as an LPN/LVN in a clinic setting preferred

  • Complies with all relevant professional standards of practice

  • Participation and completion of Amergis' Competency program when applicable

  • Current CPR if applicable

  • TB questionnaire, PPD or chest x-ray if applicable

  • Current Health certificate (per contract or state regulation)

  • Must meet all federal, state and local requirements

  • Successful completion of new hire training as applicable to job site

  • Understand patient confidentiality and HIPAA requirements

  • Ability to effectively elicit/provide information to and from appropriate individuals (including, but not limited to, supervisors, co-workers, clients) via strong communication skills; proficiency in the English language is required

  • Computer proficiency required

  • Must be at least 18 years of age
