The Arc Erie County NY Care Manager in Williamsville, New York

The Arc Erie County New York has been supporting children and adults with disabilities, and their families in achieving their desired quality of life by creating opportunities through comprehensive services for over 65 years.

Our Position: We are currently seeking a full-time (40 hrs) Care Manager to provide oversight to the Respite Service people we support.

This position also involves:

  • Provides comprehensive care management to the eligible recipients of Health Home services, Comprehensive Care Management, Care coordination & health promotion, comprehensive transitional care, individual & family support & referrals to community and social support services.

  • Assists service recipients & families to access care in the most appropriate setting.

  • Conducts a comprehensive health assessment with service recipients using NYSDOH assessment tools (CANS- NY & UAS).

Our Schedule: Monday-Friday 8:00am-4:30pm. No nights, no weekends and no holidays!

Our Rate of Pay: $19.00-$22.00 per hour based on experience.

Our Requirements:

Master’s Degree in Social Work, Psychology, or a related Human Service Field PLUS one year of relevant experience.

OR

Bachelor’s Degree in Social Work, Psychology, or related Human Service Field PLUS two years of relevant experience

OR

Licensed as a Registered Nurse PLUS two years of relevant Experience.

PLUS

Must become certified in CANS-NY 0-5 year old AND 6-21 year old within 30 days of hire.

Position Summary:

This position involves overall accountability for the daily coordination of individualized child/adolescent care needs. Provide services to child/adolescent up to the age of 21. Conducts a comprehensive health assessment with child/adolescents using NYSDOH assessment tools Child and Adolescent Needs and Strengths (CANS-NY) & Uniform Assessment System (UAS-NY). Assist child/adolescent & families to access care in the most appropriate setting. Sets child/adolescent goals, monitors and reports status of goals to all care system providers.

Ensures that a holistic approach is used and that goals for all dimensions of health are met. Collaborates with all providers within service recipients’ care system including service recipients’ family/legal representative to ensure that child/adolescent care is coordinated, including accompanying documentation and referrals, appropriate community referrals, and completed family/child/adolescent education and linkages.

Reporting Relationship:

Receives direct and on‑going administrative supervision from the Coordinator of Care Management.

Job Duties and Responsibilities:

  • Provides comprehensive care management to the eligible recipients of Health Home services, Comprehensive Care Management, Care coordination & health promotion, comprehensive transitional care, individual & family support & referrals to community and social support services.

  • Assists service recipients & families to access care in the most appropriate setting.

  • Conducts a comprehensive health assessment with service recipients using NYSDOH assessment tools (CANS- NY & UAS).

  • Assists service recipient and family in identifying and accessing community resources to satisfy service recipients’ needs.

  • Assists Health Home/Early Intervention Individuals through NYEIS in the Health Commerce system with linkage to appropriate services, holding Individualized Family Service Planning Meetings and transition to CPSE.

  • Assists families and children in the acquisition and maintenance of public benefits; financial, educational, social services.

  • Provide education to the family and child concerning the nature and desired effect of care management services, along with information concerning conditions being treated.

  • Ensures responsible transition of client services into and out of health home care, between child and adult health homes, and between inpatient and community care as appropriate.

  • Develops a comprehensive, family driven youth guided plan of health care. Sets service recipients’ goals, monitors and reports status of goals to all care system providers.

  • Completes service recipients’ care plan with all providers within service recipients’ care system including service recipients’ family/legal representative. Ensures plans are accurate and completed in a timely manner.

  • Documents all case activity in the respective health home electronic medical record, including outreach, consent development and assessment, plan development, client progress and transition arrangements.

  • Ensures continuity of care by collaborating and maintaining regular contact with all members of the service recipients’ care system.

  • Conducts and convenes family meetings to review progress, update plans of care, and confirm continuing client eligibility.

  • Employs a culturally-sensitive, person-centered planning approach to service delivery

  • Participates in mandatory and optional training, including but not confined to: CQL/POMS, CANS-NY family driven youth guided, trauma informed care, IT, LGBT, safety in the community work and meeting management; participate in regular supervision.

  • Must achieve a score of 70% or higher on the CANS-NY examinations annually and within 30 days of hire.

  • Performs other duties as assigned by the Coordinator of Care Manager.

Qualifications and Education Requirements:

Master’s Degree in Social Work, Psychology, or a related Human Service Field PLUS one year of relevant experience.

OR

Bachelor’s Degree in Social Work, Psychology, or related Human Service Field PLUS two years of relevant experience

OR

Licensed as a Registered Nurse PLUS two years of relevant Experience.

Must become certified in CANS-NY 0-5 year old AND 6-21 year old within 30 days of hire.

Relevant experience in serving children and families in child welfare, developmental disabilities, behavioral health, health care, early intervention or social services.

Demonstrate interpersonal and communication (verbal and written) skills, including ability to develop care plans with well-defined goals and objectives. Ability to interact with diverse people including health home members, their families, and community service providers.

Ability to work in an effective and focused manner when self-directed and beyond immediate oversight. Demonstrated efficiency with organizational and time management skills.

Ability to learn new computer applications.

The ability to bend, stoop, squat, kneel, sit, reach, stand, walk, push, pull, and climb flights of stairs. Must have the ability to see, hear, listen, and speak clearly.

Must have good visual, oral and written communication skills to observe and assess needs and maintain documentation of services provided.

Receive supervision instruction in a positive manner. Adhere to and administer applicable federal, state, county and agency procedures, rules and regulations. Maintain a professional attitude and appearance at all times.

  • Equipment Operation:

The ability to operate business equipment; telephones, fax machines, computer hardware and software, including applications and programming, copiers, calculators, etc.
