Emergent Health Partners Integrated Care Paramedic in United States

Collaboratively working with a healthcare team, the Integrated Care Paramedic (ICP) will conduct comprehensive assessments for patients with acute and chronic medical issues in various settings, including their homes. The ICP will provide both urgent and routine assessments, conduct thorough physical examinations, offer appropriate care and interventions, and facilitate coordination with the patient's healthcare team to establish a safe and effective care plan. Additionally, the ICP will extend support to patients and fellow team members, focusing on preventative care and wellness to help patients maintain their current setting and minimize avoidable hospitalizations.

Essential Duties:

· Work closely with team members, providers, and external agencies to safeguard patient health and well-being.

· Conduct a variety of medical assessments and interventions under medical director guidance and care team coordination.

· Manage an established plan of care, evaluate efficacy of interventions, provide effective coaching and advocacy to progress goals forward.

· Provide disease specific education with patients, family, and/or caregivers.

· Review medical records, treatments, screenings/tests, and effectively coordinate with members of the patients care team.

· Triage a variety of medical concerns and establish an effective plan to ensure patient needs are met.

· Maintain precise documentation through written reports using various Electronic Medical Records.

· Attends training sessions as required to maintain certification and develop additional community paramedic skills.

· Educate enrolled patients, TANDEM365 Associates, and external agencies/providers about services.

· Fulfill any other assigned responsibilities.

Work Environment:

· While performing the duties of this job, the associate is required to stand, walk, use hands and fingers, handle or feel objects, tools, or controls; reach with hands and arms; climb stairs; balance; stoop, kneel, crouch or crawl; talk or hear; taste or smell. Specific vision abilities required by the job include close vision, distance visions, color visions, peripheral vision, depth perception and the ability to adjust focus. The associates must meet current physical requirements established by HVA/EHP. #CB

Required Qualifications:

· Proficient knowledge of emergency care, common illnesses, symptoms, and medical terms.

· Capable of making sound decisions independently and collaboratively, even in high-stress situations.

· Skilled in preparing comprehensive written reports.

· Effective time management and organizational abilities.

· Competent in creating educational materials and delivering training as needed.

· Current Paramedic Certificate for the State of Michigan, or current National Registry -- must be Michigan state certified before hire date.

· Current certification in CPR (Cardiopulmonary Resuscitation) and ACLS (Advanced Cardiac Life Support)

· Valid State of Michigan motor vehicle operating license, with continued safe driving history

· Eligible for Michigan Community Paramedic endorsement by possessing a certification of completion of a Community Paramedic course from an accredited University or Training Facility, and have successfully passed, and hold current certification from the International Board of Specialty Certification (IBSC) CP-C exam
