Human Services Program Consultant – HIV/AIDS Program

Your Specific Responsibilities :

Reporting to the HIV Surveillance Program Manager, this position serves as the NIR (No Identified Risk) Coordinator and assists with Linkage to Care to assist clients following positive confirmatory HIV test results or a newly reported Mama Bear (HIV positive pregnant client).

Responsible for coordinating, providing technical assistance, and implementing activities associated with identifying risk information on reported Human Immunodeficiency Virus (HIV) and Acquired Immune Deficiency Syndrome (AIDS) cases. Contacts reporting providers to ascertain risk information. Requests and reviews medical records from public and private hospitals and other reporting sites, either in person or through Electronic Health Records (EHR). Investigations require interviewing individuals meeting program inclusion guidelines and assist in locating and providing follow-up counseling and referrals to individuals infected with HIV.

The position also identifies HIV+ individuals that are not in medical care, not taking antiretroviral (ARV) medications, or have tested positive for HIV but have not been notified of their status; they will assist Linkage to Care in contacting individuals that are not in medical care, taking ARVs, or that have tested HIV+ and not been notified of their status and determine how to best link them into care. Arranges appointments for interviews at patient care clinics, reporting facilities, private physician’s offices and hospitals, or otherwise conducts interviews over the phone or in-person. Conduct field activities to confirm that client attended their medical appointments. Conduct interviews and identify barriers to care and work with clients until they are engaged in medical care.

Duties include (but not limited to):

  • Performs HIV/AIDS passive and active surveillance duties on an ongoing basis.

  • Scanning and processing of incoming laboratory tests or ICD-10 lists.

  • Record searching patent names, on incoming labs & Theradoc/SOAP hospital notes, in the eHARS/Axiom Pro database to note whether the case is HIV, AIDS, NIR (missing a risk), or has additional updated information needed to be added to the case in eHARS.

  • Scanning or electronically reporting new and update HIV and AIDS case report forms to the State Health Office (SHO).

  • Quality data checking the monthly case reports and laboratory tests in Axiom Pro to ensure the scanning of the documents is complete and accurate, shredding documents and ensuring confidentiality of data/papers.

  • Answering phones and obtaining new HIV/AIDS case report information from providers that call into the Surveillance office.

  • Documents and coordinates reclassification of NIRs and prepares and submits required case report forms that are to be scanned or electronically sent to the State Health Office on a weekly basis, as it relates to risk exposure.

  • Identifies providers with high NIR rates and provides appropriate feedback and technical assistance as needed.

  • Coordinates NIR workshops and in-services for registered Counseling/Testing sites with the help of the local Early Intervention Consultant (EIC).

  • Completes designated NIR Questionnaires

  • Assist in investigating and documenting the preferred methods to communicate with clients and secure any essential consent forms authorizing communication between the clients and department staff.

  • Conducts routine matches (record searches) with the STARS database, TB database, HMS database, ADAP database, CAREWare, and Case Management Agencies to help identify risks and expanded risk information.

    Required Knowledge, Skills, and Abilities :

  • Ability to establish and maintain effective working relationships with others.

  • Ability to conduct interviews of HIV/AIDS patients to identify risk factors with documented consent.

  • Ability to conduct project related surveillance functions for the purpose of reclassifying cases which have been reported as “NIR” (i.e. no identified risk).

  • Ability to apply applicable rules, regulations, policies and procedures relating to HIV Surveillance/LTC/ADAP/STD.

  • Ability to perform venipuncture or willingness to learn within six months of hire.

  • Knowledge, skills and abilities, including utilization of equipment, required for the position.

  • Knowledge of basic computer software and ability to enter data in computer.

  • Knowledge of clerical functions & procedures (e.g. phone etiquette, filing and good communication skills).

  • Knowledge of the methods of data analysis and collection.

  • Skills in coordination with multiple Divisions/Programs/Groups; as well as other agencies and entities

    MINIMUM QUALIFICATIONS:

  • High-school diploma

    PREFERRED QUALIFICATIONS:

  • A Masters in Public Health degree, preferred

  • A bachelor's degree from an accredited college or university with a major in one of the biological sciences and/or one year of professional biological experience in a field or laboratory program; or

  • 1-2 years of work experience in Department of Health disease investigation and surveillance
