About Us: ALM Hopewell Center provides comprehensive mental health services to children and adults in the northern and central parts of the City of St. Louis and North St. Louis City. A professional mental health staff serves a growing caseload of 2,800 patients, of which greater than ninety-five percent are chronically mentally ill and suffering from major psychiatric disorders. Hopewell Center operates a Psychosocial Rehabilitation Center and a Comprehensive Psychiatric Rehabilitation Center for adults and youth. Many services are provided at schools. Emergency services, including a crisis bed and mobile outreach, are available twenty-four hours per day. A multi-disciplinary team of psychiatrists, psychologists, nurses, counselors, social workers, educators, and support staff work diligently to carry out the mission of providing comprehensive care to patients and their families. The Housing Liaison is tasked with leveraging the existing strengths of the agency to implement effective outreach and engagement of clients experiencing housing crisis. Create a powerful network of experts to assist with resolving housing related problems in the community and statewide. Provide assistance in the case management process to resolve housing crisis issues. PRIMARY RESPONSIBILITIES AND DUTIES • Outreach to individuals who are not currently engaged in CPR or CSTAR services who are in housing crisis or experiencing homelessness. • Refer and transport (as needed) individuals to initial appointments to establish services for employment, behavioral health, substance use, physical health, or other mainstream benefits. • Directly contact potential landlords establishing relationships to make referrals for unit occupancy. Maintain rapport with landlords . • problem solve tenant issues. • Receive referrals from Continuum of Care programs to assist individuals with housing stability. • Assist tenants with the transition process and readiness to move on to other permanent housing options. MINIMUM EDUCATION, TRAINING AND EXPERIENCE: A Bachelor's degree in Psychology, Social Work, or Counseling and/or 3 years of relevant case management experience in housing. Mental and behavioral health experience preferred. CERTIFICATION OR LICENSE REQUIRED: SSI/SSDI Outreach, Access, and Recovery (SOAR) certification or willing to obtain
