Position: Physical Therapist
Department: Home Health Care
Reports to: Home Health Services Administrator

Salary Range: depending on education and experience
Including full Health Care benefits, Retirement and 224 hours annually of Paid Time off

GENERAL DESCRIPTION
The Physical Therapist will participate in the in-home care of patients by assessing and evaluating the capabilities and disabilities of patients in order to establish a plan of care for each patient in accordance with physician ordered treatments.
ESSENTIAL JOB FUNCTIONS
Complete initial and ongoing comprehensive assessments
Provide interventions and therapeutic treatment in accordance with the physician certified plan of care
Establish, teach, implement and assess a treatment plan according to physician’s orders, reevaluate the plan periodically, according to the patient’s needs.
Develop long and short term goals for each treatment plan
Perform an accurate OASIS-C timepoint(s) according to CMS guidance
Assess changes in the patient’s status since the last visit/treatment day, use critical thinking skills to report clinical changes to nursing staff
Submit documentation of treatment and skilled intervention provided
Participate in educating the patient and the family, and other caregivers to promote patient progress toward mutually established goals.
Participate in the development of a total plan of care
Regular and predictable attendance
REQUIRED SKILLS
Graduate from an accredited Physical Therapy Collegiate Program
Registered professional physical therapist with license in the State of Missouri
Prefer two years minimum experience
Possesses and maintains current CPR Certification
Must be a licensed driver with an automobile that is insured in accordance with state requirements and is in good working order.

Basic computer skills and knowledge of Microsoft office
Ability to communicate appropriately and effectively with patients and their family, physicians, staff and other healthcare professionals
Ability to manage time effectively
Ability to maintain integrity
Ability to read and comprehend medical instructions given from written or verbal sources
Ability to work with minimal or no supervision
Ability to use diplomacy and tact when dealing with problems.
