What You Provide Us:
- Meets with patients for one-on-one support and guidance
- Assists patients with developing recovery plans, coping methods, and rehabilitation strategies with patient and behavioral health team
- Monitors patient\'s behavior, schedule, medication, and activities to report back to behavioral health team
- Liaising with family members and medical professionals regularly
- Generates recovery reviews and performance documents to track the rehabilitation process
- Assists with arranging medical appointments, therapy sessions, support groups, and other resources to aid the patient as indicated by the patient
Must Haves:
- High School Diploma or GED required
- Completed Peer Support Training with a valid certificate
- Experience in community engagement, with emphasis on building & maintaining partnerships
- Valid Fingerprint Clearance Card
- Any combination of experience and education that provides the required skills may be considered
