Under the guidance of the Nursing Supervisor, you will partner closely with clinical service providers, the medical team, and other staff members to address individual's needs related to medical services and medication management. Engage patients with care and support without requiring close oversight. You have the ability and passion to work with patients that have high resistance and denial or impairment due to chronic mental illness and/or substance abuse disorder dependency. Receive, manage, and route phone calls, messages and/or concerns. Maintain relationships with clients and family members by answering medical, questions, and guiding them through our policies and procedures. You enjoy being the first point of contact for patients by capturing necessary information to help facilitate medical appointments, and ensure that patients receive warm hand offs to providers. Verify patient information by interviewing patients, recording medical history, confirming purpose of visit, vitals, and any appropriate diagnostic testing. Working in a team based environment is important to you. Partner closely with nursing and other staff members regarding follow-up appointments, medical refills, experiencing symptom changes, etc. Assist in maintaining provider caseloads by providing outreach to patients that have not been seen within 90 days to re-engage patients via 10 day closing letter and phone call. Work with hospital liaison and nursing staff to coordinate med somatic appointments with appropriate providers at hospital discharge within a 2 week time period to ensure timely medication refills. Complete prior authorizations with MCO's, Medicaid, Medicare, and private insurances for patient medications to help expedite clients access to medication. You have.

  • HSD/GED
  • Previous experience working as a Medical Assistant
  • Understanding and previous working knowledge of HIPPA
  • A valid driver's license, with the ability to maintain auto insurance
  • CPR/First Aid Certification or willingness to obtain one
  • Ability to use various computer apps including Microsoft Office, our HRIS, EHR, and other online systems, with multiple logins often simultaneously
  • Willingness to submit background check and fingerprinting

Preferably.

  • Previous experience working in behavioral health field while working with individuals that have a primary mental health diagnosis and/or co-occurring disorders
  • CMA (Certified Medical Assistant)
