Hourly Pay Range (Minimum - Midpoint - Maximum):

$68.302 - $88.132 - $107.961

The stated pay scale reflects the range that Sharp reasonably expects to pay for this position.  The actual pay rate and pay grade for this position will be dependent on a variety of factors, including an applicant’s years of experience, unique skills and abilities, education, alignment with similar internal candidates, marketplace factors, other requirements for the position, and employer business practices.

What You Will Do

Coordination of the interdisciplinary, hospital-wide Sharp Patient Safety Program. Responsible for risk identification, prevention and mitigation of patient safety events through the application of expertise in safety and reliability science. Leads patient safety initiatives. Coaches leaders, staff and physicians in fostering and sustaining a strong safety culture.

Required Qualifications

  • Bachelor's Degree in Nursing

  • 5 Years Clinical experience (ICU or ED preferred)

  • Experience analyzing and interpreting patient safety-related data to recommend specific improvement strategic priorities

  • Experience interacting, in oral and written forms, with staff, leaders, physicians, patients and families.

  • California Registered Nurse (RN) - CA Board of Registered Nursing

Preferred Qualifications

  • 3 Years Experience in healthcare risk reduction/quality/safety

  • Leadership experience

  • Certified Professional in Patient Safety (CPPS) - Certification Board for Professionals in Patient Safety -PREFERRED

Other Qualification Requirements

  • Certified Professional in Patient Safety (CPPS) is required within 2 years of hire. The department is responsible for tracking and maintaining this certification.

  • EducatorTrains and coaches leaders and staff on the use of RL Solutions.Teaches CARE Program, TeamSTEPPS, Just Culture at Sharp HealthCare system level and mentors other at the entity level.Coordinates annual Patient Safety Summit program and other patient safety educational programs at assigned entity.High Reliability Organizing (HRO) trainer and mentor at entity level.Creates and disseminates Safety Alerts and Great Catches.

  • Meeting attendanceThe Patient Safety Program Coordinator is a required member at system and entity level meetings related to patient safety.

  • Process improvementConducts environmental and patient safety rounding to identify and mitigate risk concurrently.In collaboration with front-line managers, associates and physicians, responsible for the analysis of clinical process opportunities and the development of strategies to maximize safe practices.Responsible for coordinating the deployment of process improvements that support the reduction of risk and medical/clinical errors and other factors that contribute to unintended adverse patient outcomes.Lead and mentors others on the use of A3, Failure Mode Effect Analysis (FMEA), and other process improvement tools to improve patient safety.Monitors trends in Patient Safety Indicators (PSIs) and Hospital Acquired Conditions (HACs) and coordinates action plans to improve performance.Utilizes best practice strategies and methods within safety science and PI to improve all aspects of patient safety.

  • Professional competencyCertified Professional in Patient Safety (CPPS) is required within 2 years of hire. Documentation and tracking of this certification is the responsibility of the department and will be maintained by the department.

  • Program coordinationCollaborates at the system and entity level to plan, develop, implement and evaluate patient safety program elements:* Culture of Safety Survey: Resource to leaders for promoting survey, interpreting results and creating targeted action plans.* Safety Assessments.* Reporting of events and near misses: a) Daily review of safety events; b) Identification of events for Patient Safety Event Review Team (PSERT); and c) Prepare reports for external reporting.* Cause Analysis, Response and Evaluation (CARE) Program: a) Safety Debriefs; b) PSERT Processes; c) Cause Analysis: i) Apparent Cause Analysis (ACA): Leads and mentors others; and ii) Root Cause Analysis (RCA): Leads and mentors others; d) Corrective Actions to Prevent Recurrence; e) Action Plan Monitoring for long-term sustainment; and f) Spread and adoption of best practices.* Safety Event Classification: a) Serves as expert and mentors others; and b) Tracks case laws to help maintain inter-rater reliability and consistency over time.* Common Cause Analysis: a) Categorizes events accurately; and b) Analyzes and interprets results and identifies corrective action plans.Coordinates defective device response in collaboration with Clinical Effectiveness, Biomed, Safety Officers, Regulatory, Legal, Pharmacy, Supply Chain and other stakeholders.Monitor and comply with updates/changes in regulatory standards and timely communication of changes to relevant depts.

  • Leadership, mentorshipParticipates and role models during huddles.Works collaboratively with leaders to help local leaders identify and hardwire behavioral norms (briefs, debriefs, learning boards, TeamSTEPPS skills) that promote culture of safety.Applies expert knowledge of safety to mentor leaders in the organization through appropriate use of resources to achieve and/or maintain top decile performance in all safety metrics.Evaluates efficacy and safety of products, devices, and technology related to patient safety.Review and contribute to improving key policies and processes related to risk prevention, identification, reporting, mitigation, and correction.Collaborate with and serve as a patient safety expert consultant to the medical staff, leadership team and system quality teams including assisting with identification of relevant best safety practices.Mentors safety coaches in order to enhance learning from errors and defects, to promote second-order problem solving and to advance proactive team-oriented communications.

Knowledge, Skills, and Abilities

  • Knowledge of regulatory requirements including: TJC, CMS, CDPH, and IMQ required.

  • Operational knowledge of computer software applications, including MS Office (Excel, Visio, Word, Powerpoint), Publisher and Cerner is required.

  • Knowledge of safety and reliability science, Lean Six Sigma, Just Culture and infection prevention is preferred.
