Center for Life Management Therapist, Collaborative Care (intellectual disabilities and/or traumatic brain injury) in Salem, New Hampshire

Center for Life Management (CLM) provides the area's most comprehensive array of mental health services. Our Collaborative Care Program is an innovative approach to healthcare that encompasses the integration of mental, physical and behavioral health needs. Working together as a multi-disciplinary team, internal (CLM) and external (area agencies) work as one entity to support individuals with co-occurring mental health needs and intellectual disabilities (IDD) and/or traumatic or acquired brain injury (TBI, ABI). We are seeking a Master's Level Therapist (MSW, LICSW, CMHC, LCMHC) to join our incredible Collaborative Care Team. The Collaborative Care Clinician provides outpatient community-based support, expressive therapy (play, art, interactive modes of treatment), diagnostic assessment, treatment planning, and crisis management to help client’s improve symptoms and behaviors that otherwise interfere with daily functioning and personal wellness goals. This is a full time, 35 or 40 hour per week position. This is not a fully remote position and requires some in office days.

Major Functional Areas and Description of Duties of the Intellectual Disabilities Therapist

  • Provide individual diagnostic assessment to determine client’s strengths, needs, resources, and objectives;

  • Formulate treatment plans specifying presenting problems, objectives, interventions, and desired outcomes;

  • Complete assessments for eligibility determination for State funded services;

  • Provide individual and group therapy in the office and/or remote via telehealth;

  • Provide counseling and therapy services in the home and community-based settings as deemed appropriate for the objectives, interventions and desired outcomes identified in the client’s treatment plan;

  • Develop, apply, and reinforce coping strategies to reduce and/or ameliorate symptoms and behaviors that interfere with functioning and goal achievement;

  • Provide counseling support to families and natural supports as needed to educate and promote how to best encourage the client’s resiliency and progress toward treatment goals;

  • Provide psycho-education using the evidence-based practice of Illness Management and Recovery (IMR) as needed to support goal and coping skill development;

  • Provide crisis management and intervention when clients experience acute exacerbation of symptoms to ensure their safety within the home and/or community;

  • Adhere to and uphold the practices specific to the Continuum of Collaborative Care Model specific to support the authenticity of program implementation (training to be provided on model and adherence there of);

  • Work collaboratively with other treatment team members, natural supports, and external providers to ensure a wrap-around approach is provided for continuity of client care;

  • Develop training opportunities in an area of expertise to educate internal and external colleagues;

  • Provide set hours of billable service each week (eligible for incentive pay upon exceeding productivity expectation);

  • Engage in appropriate consultation meetings and committees, both internally and externally, as determined by supervisor;

  • Function professionally and in a manner that protects the integrity, confidentiality and rights of all clients;

  • Execute all required services and timely documentation in accordance with agency policies and federal/state standards;

  • Maintain professional memberships, licenses, accreditation, certifications, etc.;

  • Represent CLM in a professional and appropriate manner in all settings;

  • Perform all other duties as assigned by supervisor.

    Qualifications and Skills

  • Master’s degree required in mental health field (social work, clinical mental health counseling, psychology, expressive therapies, intellectual disabilities, marriage and family, etc.);

  • Licensed Clinical Mental Health Counselor (LCMHC), Licensed Independent Clinical Social Worker (LICSW), Licensed Marriage and Family Therapist (LMFT) or licensed eligible master's level clinician preferred;

  • Experience working with co-occurring disorders preferred;

  • Ability to engage adults 18 and older in interactive play and therapy modalities to engage clients in therapeutic processes with a range of developmental needs;

  • Ability to exercise sound clinical judgment in routine, urgent, and emergent situations;

  • Proficient computer skills, specifically with all Microsoft office applications;

  • Ability to document effectively and efficiently in course of clinical work;

  • Ability to navigate an electronic health record;

  • Ability and desire to work as part of a clinical team while also able to be self-directed and motivated with minimal supervision;

  • Ability to acquire and effectively practice new clinical skills as needed according to evolving agency needs/services;

  • Demonstrate effective communication skills, organizational skills, and ability to multi-task.

    Physical Demands and Working Conditions

The work will be performed in the community, client’s home, office or remotely via telehealth depending on the treatment objectives and interventions. Manual dexterity is needed to use computers. The ability to sit for extended periods of time is required. Reliable internet access, transportation, a clean driving record, and proof of auto insurance coverage is required.
