The Title | | | X-Community Care Navigator | | | supports this mission by working | | | alongside the program supervisor | | | to provide a combination of | | | administrative and patient care | | | support to patients that qualify | | | for Title X services. Title X | | | Family Planning Programs aim to | | | improve access to family planning | | | and related preventative health | | | services. The Community Care | | | Navigator will engage with | | | medical providers and patients to | | | monitor Title X funded services, | | | and engage in the retention and | | | outreach of Family Planning and | | | Preventative Healthcare | | | qualifying patients.
