Description

SUMMARY: Utilizes the nursing process to provide quality age appropriate nursing care to patients that is consistent with organizational philosophy, goals and policies and procedures.

ESSENTIAL DUTIES AND RESPONSIBILITIES: Include but are not limited to, the following. Other duties may be assigned.

RESPONSIBILITIES AND STANDARDS

Responsibility I

1.0 Provides for the delivery of patient care through an ongoing patient assessment with

identification of pertinent problems and needs.

1.2Documents changes in patient's condition reflective of an on-going patient assessment.

1.3Communicates significant changes in the patient's condition to the health care provider (HCP) in a timely and appropriate manner.

1.4Demonstrates the knowledge of significant diagnostic tests and intervenes appropriately.

1.5Demonstrates the knowledge of the principles of growth and development over the life span.

1.6Interprets the appropriate information needed to identify each patients requirements relative to his or her age-specific needs.

1.7Provides patient care needed as per organizational policies and procedures.

Responsibility II

  1. 0 Assumes responsibility for development and implementation of a realistic Nursing Plan of

Care for each patient and for ongoing evaluation and revision of the plan each shift.

Development of the patient's plan of Care should reflect collaboration with the patient

and/or significant others whenever possible.

2.1Initiates a Nursing Plan of Care and identifies nursing diagnoses, problem list and interventions within 24 hours of admission.

2.2Reviews, documents and updates the patient’s Plan of Care every shift in the EMR to include goals of care and target dates.

2.3Initiates and documents discharge plan on day of admission in collaboration with the HCP, patient and/or significant others, and follows through with appropriate interventions.

2.4Initiates patient education and a teaching plan on the day of admission in collaboration with the patient and/or significant others. Initiates, ongoing appropriate teaching, demonstration and interventions and documents in the medical record.

Responsibility III

3.0Demonstrates the skill and judgment necessary to implement HCP orders, nursing procedures and nursing interventions. Implements HCP orders accurately and promptly using nursing judgment

3.1Performs and assists HCP with procedures as necessary and documents the patients response to these procedures in the EMR

3.2Implements and performs nursing skills in an efficient and competent manner.

3.3Provides ongoing nursing care as observed by the Nurse Director or designee and documents patient's response to this care in the EMR.

3.4Continuously monitors the patient's ability to safely meet his own ADL needs, instructing and providing appropriate assistance, when necessary.

3.5Recognizes potential emergency/life threatening situations and initiates appropriate actions.

3.6Demonstrates knowledge of therapeutic action, side effects and interaction of medications.

3.7Demonstrates competency in administration of blood and blood product therapy and knowledge of the Bloodless Care program when indicated.

3.8Adheres to Infection Control, Patient Safety and Quality Systems policies and procedures at all times.

Responsibility IV

4.0 Coordinates and promotes continuity of patient care through clear communication and

collegial working relationships.

4.1Documents patient assessments ongoing and reassessments in a logical sequence; accurately reflecting the patient's condition. Identifies and documents the patient's response to care, treatment and medications.

4.2Collaborates and communicates with the patient’s HCP in a constructive and professional manner highlighting any change in condition or any adverse reaction to medications or treatments.

4.3Provides a timely, concise, pertinent handoff report utilizing designated tool in EMR to oncoming shift and ancillary personnel. The report must clearly identify any problems or change in condition and emphasize any patient problems that require follow-up.

4.4Makes appropriate assignments of patient care based on patient care needs.

4.5Oversees delegates and prioritizes care provided by unlicensed assistive personnel and frequently evaluates their care throughout the shift.

4.6Communicates effectively and frequently with health care team members to share information and solve problems in a positive, result-oriented manner.

4.7 Participates as a preceptor in unit orientation as requested.

4.8Compiles a thorough discharge.

4.9Lends knowledge and/or assistance to other nursing units or departments when requested with a positive and cooperative attitude.

4.10Functions, after successful completion of probation and the appropriate orientation, in a competent manner as a charge nurse.

Responsibility V

  1. 0 Demonstrates knowledge of evidence based practice and current trends and practices in

the health care field.

5.1Utilizes available educational materials, programs, nursing literature and resource personnel for professional development.

5.2Completes all mandatory annual requirements (physical, competencies and education).

5.3Attends monthly unit staff meetings, pre-shifts and/or reviews minutes according to unit policy to maintain knowledge of hospital policy changes/hospital activities.

5.4Remains flexible and responsive when unexpected changes occur in staffing or workload.

5.5Functions, after successful completion of probation and the appropriate orientation, in a competent manner as a charge nurse.

Responsibility VI

6.0 Utilizes material, equipment and time in a safe, therapeutic and cost effective manner.

6.1Organizes workload to complete responsibilities within assigned shift.

6.2Maintains supplies as needed for patient care.

6.3Utilizes available equipment knowledgeably and effectively; reports malfunctions and other potentially hazardous situations as they arise to Nursing Manager and/or appropriate department.

6.4Operates computer as needed in a competent and professional manner.

Responsibility VII

7.0 Participates in unit Quality Systems activities.

7.1Utilizes the results of Quality Systems audits and activities to modify and/or enhance clinical practice.

7.2Documents all discrepancies and patient incidents; including, adverse reactions, Rapid Responses and Codes according to hospital policy.

7.3Reports all discrepancies and patient incidents to the Nurse Director or designee and the patient's HCP in a timely and comprehensive manner.

ResponsibilityVII

8.0For all managers and supervisors who are responsible to track time & attendance, the following responsibility will apply: Accurate and timely approval of paid employee hours through API

8.1For all managers and supervisors who are responsible to manage budgets, the following responsibility will apply: Monitors expenses according to approved budget and provides timely variance report analysis.

Responsibility IX

9.0 Guest Relations: Demonstrates regard for the dignity and respect of all patients, family

members, visitors and medical center personnel as defined in the philosophy of Vassar

Brothers Medical Center

9.1Displays a caring and courteous attitude and represents the medical center in a positive manner to all persons noted above.

9.2Promotes and contributes positively to intra-departmental and inter-departmental relationships.

9.3Maintains the confidentiality of employees and departmental information with no infractions

9.4Adheres to dress code policy and wears a VBMC picture identification badge, 100% of the time.

9.5Demonstrates willingness to assist co-workers and/or to accept additional assignments as requested to support the department's efficiency as observed by the Supervisor.

9.6Responds to all Supervisors’ requests for information and assists in a timely and courteous manner.

9.7Demonstrates a high level of mental and emotional tolerance and even temperament when dealing with people; uses tact, sensitivity and sound judgment at all times.

JOB QUALIFICATIONS:

Education:BSN Preferred.

Graduate from accredited school of nursing

Experience:Must have at least one year of hospital medical-surgical nursing experience. Any RN with less than one year of hospital medical-surgical nursing experience or any new graduate RN will be hired into the hospital new graduate orientation program approved by the ACNO.

License:Current New York State Registered Nurse License BLS Required
