Job Description:

Clinical

The Nurse Practitioner shall provide the following clinical services to patients of Beth Israel Deaconess Hospital-Plymouth and the HMFP/JPA Cardiology Practice:

  • Manage inpatient consult service and hand-off communication to consulting service cardiologist.

  • Provide follow up patient care on inpatient consults.

  • Performing cardiology stress testing.

  • Manage outpatient clinic patients.

  • Perform a physical examination of each patient or assure that a recent physical examination of each patient has been performed prior to the arrangement of and/or provision of cardiology procedural services. Assess physical and psychosocial status of patients by means of interview and by obtaining health and medical history of patient.

  • Provide preparation, consents, and orders for scheduled cardiology procedures.

  • Arrange for necessary testing and/or consultation that may be required.

  • Communicate with the referring physician to assure continuity of care.

  • Communicates and works with NP providers, cardiologists, and other cardiology team members.

  • Manage outpatient follow-up visits and call backs.

  • Perform other duties as reasonably assigned.

  • Establish relationships with professional and/or other health-related groups within the community;

  • Share clinical expertise with professional and supportive personnel;

  • Document and report care in an accurate, timely manner using appropriate forms and Records;

  • Function as an integral member of the Cardiology team and collaborate with other health disciplines to ensure continuity and quality of care.

  • Use interpersonal skills to work productively with all levels of hospital personnel.

  • Effective verbal and written communication skills to communicate with diverse populations, including providers, patients, and families;

  • Analytical skills to evaluate patient status and healthcare procedures/techniques and to monitor quality of care;

  • Willingness to learn and remain flexible in the changing healthcare environment;

  • Ability to conceptualize, develop, and implement new ideas or systems;

  • Ability to exercise sound judgment and discretion while performing duties;

  • Ability to meet with the public outside the organization and efficiently market and promote the Cardiology practice.

Continuous Quality Improvement

  • Identifies, analyzes, and resolves patient care problems to foster significant improvement in patient care.

  • Participates in interdisciplinary continuous quality improvement activities on an ongoing basis.

  • Demonstrates a commitment to improving the quality of care provided to patients.

Education/Experience Required

  • Masters in Nursing from an accredited program

  • 3-5 years working in a hospital based cardiology environment

  • Experience with short term, solution focused, medication therapies

  • Experience working with interdisciplinary teams

  • Experience using an electronic health record

  • Must be eligible for credentialing and contracting with all insurers, including MBHP.

Registration/Certification

  • Currently licensed in Massachusetts as a APRN (without restriction).
