Responsibilities

Terros Health is hiring a Licensed Clinician/Counselor for our Health Center Director position in Phoenix, AZ

Full-Time; Monday-Friday, 8am-5pm

Location: Near I-17/ Dunlap

23rd Avenue Recovery Health Center

Arizona Licensed is Required: LCSW, LPC, LISAC, LMSW, LAC, or LASAC in Good Standing

Offering a Highly Competitive Compensation ($92K-$100k Depending on Licensure and Experience)

The role of the Health Center Director is to inspire change for life in the patients served in accordance with Terros Health’s values of integrity, compassion, and empowerment in an outpatient integrated care setting. This is demonstrated by overseeing processes to help patients achieve their whole health recovery goals, as well as achieving success in the seven core outcome measures. The seven core outcome measures include decreased psychiatric and medical inpatient visits, decreased emergency department and urgent care visits, decreased arrests and incarcerations, decreased homelessness, and increased employment and Primary Medical and Psychiatric Care visits.

Additional duties include, but not limited to:

  • Serves as the clinical and administrative supervisor of an outpatient clinic working with individuals diagnosed with a Serious Mental Illness, or General Mental Health diagnosis under Court Ordered Treatment.

  • Provides clinical oversight and supervision, training and mentoring for direct and non-direct reports providing services to clients served.

  • Ensures clinical quality standards are met or exceeded by direct and non-direct reports.

  • Evaluates member care, and promotes an environment of compassionate, high quality service to enrolled individuals.

  • Creates an environment that promotes an outstanding, member-focused, healthcare experience.

  • Supervises clinical activities ensuring program services are provided in accordance with policies and procedures, local and federal laws, regulation and licensure/accreditation standards.

  • Embraces the recovery philosophy and promotes a safe recovery environment for clients.

  • Collaborates with external service providers, including medical providers, to assure best care is provided to clients.

  • Implements the policies and management procedures established by Terros Health.

  • Effectively uses data to drive health and business outcomes in a multi-team environment.

  • Responsible for the clinical care provided by the site with emphasis on coordination between clinical disciplines, coordination of clinical activities of the program, and implementation of objectives outlined in the ADHS and RBHA Provider Manual

  • Provides oversight to clinical teams to ensure that clients receive appropriate care within the least restrictive setting. This includes inpatient and residential treatment discharge plans, as well as level of services provided by the outpatient clinical team

  • Available on-call, unless otherwise scheduled in event of a scheduled absence in which coverage has been arranged

  • Pursues best practice outcomes for person with mental illness including continuing education, employment, independent housing and community tenure

  • Promotes a recovery environment, embraces and models recovery philosophy, and uses strength-based engagement strategies for all members served. Models appropriate recovery-focused language and services to all staff, both direct reports and other non-direct report colleagues. Consistently communicates Terros Health vision and mission to staff and members. Integrates the nine guiding principles for systems and services for persons with a serious mental illness into everyday clinical practice and clinic operations

  • Ensures documentation in the Electronic Health Record are completed the same day Service Delivery is provided. Ensures that the staff meet minimum actual time requirements, as directed by Terros Health Leadership. Ensures RBHA metrics, including F2F, HV, Part E, ISP, ARCP, and Demographics, are met or exceeded.

  • Prepares routine and ad hoc reports as required by Senior Director, Terros Health Leadership, or the RBHA

  • Brings to the attention of the Senior Director of Recovery Clinic Services any unmet service needs identified by line staff in the treatment planning process and assists line staff in identifying interim services available to meet member needs. Communicates operational barriers, including service development/network issues, to Terros Health leadership as necessary. Works cooperatively with site staff to resolve identified issues at the site level

  • Maintains reporting and tracking mechanisms for various deliverables, including, but not limited to: service delivery, face to face and home visit contacts, treatment plans and assessment completion, inpatient psychiatric and medical hospitalization, client employment and housing status, crisis plan and demographic completion, case load ratios, court ordered treatment status reports, mental health court status reports, and Special Assistance tracking

  • Monitors and supports efforts to connect clients to physical health services, including Primary Care services, specialist services, and medical appointment follow up, and after care plans.

Qualifications

  • Master’s degree in behavioral health or related field

  • Must have at least1-3 yearsof experience in the behavioral health or integrated health care field.

  • Must have at least3-5 yearsof experience in a leadership role.

  • Licensed at the associate or independent level (LAC, LPC, LMSW, LCSW, LASAC, LISAC)in accordance with ARS Chapter 33, Article 1, under the Arizona Board of Behavioral Health Examiners preferred

  • Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members.

  • Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions.

  • Thorough understanding of clinical documentation requirements and procedures. Basic understanding of the DSM-V preferred.

  • Preferred - Electronic health record, NextGen

  • Must have ability to perform first aid or cardiopulmonary resuscitation as necessary and appropriate.

  • Must have a valid Arizona Fingerprint Clearance card or apply for an Arizona fingerprint clearance card (Level 1) within 7 working days of assuming role.

  • Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy.

  • Must pass a TB Test

  • Must be able to pass the Arizona Department of Child Safety Direct Service Central Registry Clearance process
