Summary:

• We are recruiting for an RN Program Coordinator for the Penn Huntington's Disease Center in the Department of Neurology at PAH 330 S. 9th Street. The University of Pennsylvania Huntington’s Disease Center is recognized as a Center of Excellence by the Huntington’s Disease Society of America. Located in the heart of Philadelphia, it is the only Center of Excellence in the tristate area. We provide individualized, multi-disciplinary, family-centered care for families living with HD. The staff includes physician, nurse practitioner, psychiatrists, genetic counselors, social workers, therapists, and a full-time coordinator dedicated to treating and assisting patients with Huntington’s disease as well as their families. Our team engages in ongoing collaborative research with several organizations, clinics, and laboratories.

• The RN Program Coordinator is the primary point of contact for the HDSA Center of Excellence for patients, families, outside health care professionals and Pennsylvania Hospital’s HD Interdisciplinary Health Care team. He/She will coordinate the weekly HD clinics, and assists families with problem-solving in the daily management of HD and triage issues that arise between clinic visits, such as new or changing motor, psychiatric, and cognitive symptoms, collaborating with HD team members, ancillary departments, and external facilities where our patients receive care as needed. This coordinator will remain informed about research opportunities at the center to aid research coordinators with education and recruitment. The coordinator will also assist with coordination of community educational programs and outreach to families and health care professionals. Patients and their families at risk for HD who come to Penn for treatment benefit from personalized care throughout the course of the disease. The RN Program Coordinator is responsible for managing and coordinating care amongst the multidisciplinary care team. This position will manage referral processes and new patient intake, educate patients and families on what the center has to offer, triage complex clinical needs, partner with disciplines to ensure all care is seamless throughout the patient journey. The RN Program Coordinator will identify barriers in the care of these patients and work to solve and/or improve processes. The Program coordinator will also partner with clinical resource coordinators, social workers and community organizations to ensure patients have access to all equipment, education and resources they need. The RN Program coordinator will track and monitor the clinical progress of these patients closely. Neurology and/or Psychiatry background highly preferred.

• The Program Coordinator, RN provides clinical oversight and direct patient navigation of the specific patient population through the Department. Under the direction of the Department Clinical Leadership and Division Section Chief/Departmental Chair, the incumbent will have oversight of the development and implementation and day-to-day administrative functions of the program ensuring coordination throughout the patient continuum in collaboration with associated care teams. The scope of this work includes anticipating and facilitating multimodality dynamic scheduling for patients, triage services, development and reporting of key metrics, and coordination of care across multiple specialties and services.

Responsibilities:

• Functions in an organized and time-conscious manner.

• Ensures that the department program facilitates well-coordinated, timely, compassionate, and multidisciplinary care.

• Works across healthcare disciplines to assist in coordinating patient tests, procedures, appointments, and treatments.

• Oversee and manage an office-based patient communication system to enhance patient access to care (phone, email, Penn Chart, MPM.)Identify gaps alongside the program clinical team in the program pathway and evolving needs within the program- initiate solutions and interventions to solve gaps.

• Primary communicator with vendors including but not limited to (Medtronic, Edwards, Shockwave, and M2s).

• Reviews the list of referring providers and collaborate with Penn marketing and outreach groups to detect patterns.

• Build and foster crucial relationships with referring providers to ensure there is seamless patient flow and communication.

• Be knowledgeable of insurance and work with the business office and financial service reps regarding patient issues and trends.

• Coordinates and leads multidisciplinary care conferences with internal and external stakeholders.

• Responsible for partnering with practice leadership to continuously drive access and volume metrics. Identify opportunities to improve access, i.e. scheduling, registration, collection of records, patient experience, access.

• Systemically and continually performs the functions of assessing, planning, implementing, and evaluating the care of patients.

• Promotes programmatic research goals.

• Responsible for access and volume metrics/reporting to key department and service line stakeholders.

• Collaborates with the primary clinical provider, practice care team, and service line teams to create, use, and maintain a navigational reporting structure that is able to track the progress of patients who are receiving medical/surgical services.

• Supervise the workflow of Administrative Assistants and collaborate to coordinate patient testing and visits.

• Assists patients in overcoming barriers to care including transportation, financial counseling, referrals, etc.

• Serves as patient-facing liaison between the Penn Program and referring providers.

• In collaboration with Division Section Chief/Departmental Chair & Department Clinical Leadership provides strategic development of the program & improvement in workflows

Credentials:

• Registered Nurse - PA (Required)

• Basic Cardiac Life Support (Required)

Education or Equivalent Experience:

• Bachelor of Arts or Science (Required)

• And 5+ years Nursing experience (Required)
