The role you’ll contribute:

The Transition of Care Pharmacist supports the mission of the department, hospital, and health-system by preventing, identifying and resolving drug related problems to assure patients safety, improve patients’ satisfaction, optimize patients’ outcome and minimize cost. Serves patients across the transitions of care continuum. Responsible for logistically organizing, collaborating with patients and or families or caregivers, and other health care professionals regarding discharge medications. Can perform the duties of a Clinical Staff Pharmacist.

The value you’ll bring to the team:

  • Manages and oversees pharmacy practice programs as it pertains to Transition of Patient Care. Directs patient care services, and medication systems to provide reliable, comprehensive, and individualized drug therapy that is aligned with patient needs, evidence-based best practices and regulatory standards

  • Initiates, develops and maintains appropriate allowable billing of services in collaboration with patient accounting and setting up collaboration agreements with appropriate physicians to support said agreements

  • Develops a list of high-risk disease states and discharge medication lists to target that are at risk of readmission

  • Effectively collaborates with other hospital departments, outside resources, and strategic business unit leaders to promote cooperation and efficiency in providing patient's pharmacy care needs

  • Provides departmental and institutional leadership for medical staff committees/meetings, hospital groups/task forces, and approved external programs/organizations

  • and other hospital personnel and works closely with these individuals to establish quality infrastructures utilized within the department and throughout the hospital related to transition of care for medications

  • Assists with preparation of an annual budget and manages assigned budgets and financial resources including stewardship, cost control, and restriction/surveillance programs

  • Responsible for initiating, developing and maintaining an ASHP accredited Pharmacy Residency Program and serves as a preceptor/educator for pharmacy residency and student experiential education programs, including maintaining proficiency in preceptor roles and active education/mentorship skills. Develops student, resident, and staff training experiences/competencies and creates new relationships for teaching and training opportunities

Qualifications

The expertise and experiences you’ll need to succeed :

REQUIRED:

  • Bachelor of Pharmacy, Doctor of Pharmacy, or other advanced pharmacy degree

  • State Licensed Pharmacist

  • PGY2 Preferred
