Granville Health System Registered Nurse - Emergency Department in Oxford, North Carolina

ELIGIBLE FOR A SIGN ON BONUS A professional Registered Nurse (RN), caregiver who assumes responsibility and accountability for multiple patient customers, delivering exceptional care utilizing both nursing and critical thought processes. This RN will collaborate with other members of the Emergency Services team and associated allied health providers to insure seamless and efficient delivery of care for all patient customers. This RN will insure compliance with all standards set forth by Granville Health System, North Carolina Board of Nursing, The Joint Commission, EMTALA, Hill-Burton, CMS, HHS/OIG, HIPAA, North Carolina General Statutes and other regulatory agencies for the management of all assigned personnel, systems, supplies and equipment. The RN will conduct their practice utilizing the core competencies set forth by North Carolina Board of Nursing, the American Nurses Association and the Emergency Nurses Association. The RN will process information in the Granville Health System - wide information system as a requirement of meeting full performance of assigned position responsibilities. The RN will receive a discipline specific competency based preceptor ship with validation. Duties/Responsibilities: a. TRIAGE: Accurately and consistently utilizes ESI 5 Level triage process to include all standing nursing interventions in effect. If assigned to triage, monitors waiting area for safety and customer services issues to include implementation of system rechecks on patients waiting for bed assignments. Documents appropriately in EDM electronic documentation system. b. THROUGHPUT: Facilitates flow of patients through emergency care system, communicating pertinent information to Charge Nurse (RNC) and family/significant others waiting. Insures #Pull to Full# method of bed assignment is utilized appropriately. Maintain an #MD Dispo to Depart # rate of less than 15 minutes. Communicates timely to RNC when assigned bed spaces are ready for accepting inbound patients. Timely follows all Transfer and Admission throughput processes, protocols and flow metrics. c. ASSESSMENT/REASSESSMENT: Consistently performs initial and follow-up, routine and focused, Head to Toe patient assessments on all assigned patients as appropriate, Reassessments are performed at appropriate intervals after all nursing interventions and/or as ordered by physicians. Performs initial assessment based on triage priority and acuity of patient relative to patients physical and psychosocial needs. d. INTERVENTIONS: Provides appropriate nursing interventions as ordered by physician and through utilization of established protocols based on initial systematic assessment. Administers, monitors, and documents: Medication administration, IV Therapy to include outcomes, in accordance with medical center protocols. Initiates appropriate lab and radiology studies with accuracy. e. CLINICAL COMMUNICATIONS: Communicates significant findings to physician and initiates appropriate nursing care based on department standards of practice. Maintains open communications with all ancillary staff involved with care of ED patients. Conducts all hand-offs per GHS policy utilizing SBAR technique. f. DOCUMENTATION: RN utilizes electronic documentation through the EDM system. Occasionally, RN is required to hand write some documents. These documents are signed and scanned into the electronic medical record. All patient allergies researched and documented in EDM. All nursing actions documented and timed accurately. All medications documented per BMV system. All charting performed per appropriate patient charts. All drug reactions and changes in patient#s conditions charted appropriately. All transcribed orders checked with RNC for accuracy. All hand written documentation is legible and signed per GHS policy. No unsafe abbreviations used per GHS policy. Charting adheres to characteristics of objectivity, specificity and neutrality. Belongings are accurately and appropriately accounted for and documented in patient record to include dispositions.# g. EMERGENCY RESPONSE: Demonstrates understanding of GHS EOC Code structure to included familiarization and appropriate execution of assigned duties during any / all EOC Code calls. Demonstrates competence and safe use of all emergency equipment and supplies. Functions efficiently as participating member during all Code/Trauma/Resuscitation responses. h. PATIENT EDUCATION: Provides appropriate patient education and accurate information regarding good health practices to members including access and follow-up as directed by physician. Completes all appropriate discharge teaching documentation to include obtaining all appropriate signatures. Insures discharges patients receive all appropriate copies of discharge instructions and teaching material. i. INFECTION CONTROL: Demonstrates knowledge and compliance with medical center standards that protect the health care provider and patient, reducing the spread of infection. Practices effective hand washing/sanitizing techniques routinely and in view of patients. Conducts sanitary wipe downs as appropriate and in accordance with unit/GHS policy. Practices universal precautions at all times. j. SAFETY: Plans and provides safe, effective nursing care according to established protocols. Implements safety procedures for each patient in accordance with that patient#s specific requirements. Insures beds are in lowest position appropriate for individual care, side rails are up for best practice and patient call bell system is activated and in use at all times. Identifies and reports unusual occurrences according to EOC protocols. Required:#Graduate of an accredited school of nursing with current North Carolina registered nurse license is required. A minimum of 1 year recent clinical experience in a critical care setting to include: Emergency Services and/or Intensive Care Units (mobile of facility based) preferred. #BLS, ACLS, and PALS certifications are required or within 6 months of hire date. Preferred:##NRP and TNCC are preferred.
