As a Nurse Care Manager, you will play a pivotal role in identifying, screening, planning, tracking, and coordinating personalized care plans for our consumers.

Responsibilities:

• Collaborates with consumers to assess their conditions and needs, developing, and supervising personalized care plans.
• Conducts comprehensive nursing assessments to gather physical and behavioral health information.
• Advocates for consumers and their families, linking them to resources and empowering them to improve their overall health and wellness.
• Utilizes motivational interviewing skills to empower consumers towards better health outcomes.
• Leads interdisciplinary treatment team meetings to review assessments, updates care plans, and determines follow-up frequency.
• Maintains positive working relationships with primary care providers and social service agencies, collaborating to obtain resources and meet consumer needs.
• Complieswith agency policies and procedures, ensuring documentation deadlines and productivity expectations are met.
