Trinity Health Mid-Atlantic is seeking an experienced Behavioral Health Social Worker to join our ACO team!

The ACO (Accountable Care Organization) Behavioral Health Social Worker (BHSW) serves in a collaborative role on the Care Management team, collaborating with the attributed patient population identified as high and/or rising risk via stratification and referral. The BHSW role encompasses assessment of the social determinates of health, and planning, facilitation, coordination, evaluation and advocacy for options and services to meet the comprehensive health needs of an individual, care provider, or family through communication and available resources to promote quality, cost effective outcomes. BHSW will coordinate and support behavioral healthcare for patients, care givers, and family. The BHSW will create a trusting patient relationship by assessing the needs of patients, make referrals to appropriate behavioral health resources and specialists, and provide short-term counseling and evidence-based, goal-oriented treatments as indicated. The work involves the use of motivational interviewing, assessment and clinical interventions aimed at positively affecting the physical, cognitive, and emotional functioning of patients and their families/care givers.

Minimum Qualifications:

  • Must possess a comprehensive knowledge of Behavioral Health Social Work as normally obtained through a Master’s degree in social work or a related field required.

  • LCSW licensure required.

  • One year of medical social work experience with Medicare population preferred

  • Community-based or home care social work experience preferred

  • Knowledge of discharge and transition of care planning preferred

  • Working knowledge of community resources required

  • Must have strong computer literacy skills across Microsoft and Electronic Medical Record platforms

  • Ability to work independently and remotely and as part of interprofessional team

  • Must be comfortable operating in a collaborative, shared leadership environment.
