Our client is seeking an experienced Bilingual RN Manager to oversee their Community Health Nurses and support case management. This role involves working closely with various teams to develop and execute effective care plans. Key Responsibilities:

  • Oversee the development and execution of personalized care plans to improve participant satisfaction and outcomes.

  • Facilitate and complete Interdisciplinary Team (IDT) meetings and Service Delivery Requests (SDR).

  • Serve as the manager on call during non-operational hours, handling follow-ups and initiating grievances as needed.

  • Run clinical delivery reports and represent Community Health Nurses in IDT meetings.

  • Allocate staffing and resources effectively to ensure seamless operations.

  • Maintain ongoing communication with IDT and relevant parties to coordinate care and support optimal health status for participants.

  • Collaborate with clinicians to review and ensure accuracy of assessments using the UAS analyzer system.

  • Investigate and address incidents, ensuring timely completion of documentation according to CLHC regulations.

  • Ensure complete and accurate documentation for participants, including enrollment, medical records, and clinical notes.

  • Review participant charts for quality and address any deficiencies.

  • Coordinate care management services across sites, especially during transitions between care settings.

  • Arrange long-term care services in nursing homes, rehab facilities, and other settings as needed.

  • Monitor care management activities and adjust plans of care to meet participants' needs and goals.

  • Ensure compliance with CLHC standards and federal/state regulations.

  • Stay updated on current health-related issues, clinical practices, medications, and standards.

  • Recommend improvements in services, programs, policies, and procedures.

  • Assist HR in staff interviews, coaching, disciplinary actions, onboarding, and professional development training.

  • Evaluate work performance and competency of direct reports, providing feedback and completing annual reviews.

  • Ensure compliance with regulatory standards and organizational policies.

  • Investigate and resolve employee grievances and complaints.

  • Serve as the Director of Patient Services (DPS) for the LHCSA, as assigned.

  • Perform all other duties as assigned.

Qualifications:

  • Education: Degree from an NLN-accredited nursing school (RN, BSN required; Master's Degree preferred).

  • Experience: Minimum of two years of administrative experience in a management capacity within a certified home health agency, long-term home health care, acute care, medical-surgical, critical care, or nursing home. At least one year of experience working with frail or elderly populations preferred. Supervisory experience required.

  • License: Current active and unrestricted NYS Registered Nurse license.

  • Additional Requirements: Must be legally authorized to practice in NYS. Must be medically cleared for communicable diseases and up to date on immunizations. The salary range for this position is $115,000-$118,000/year
