Summary:

Performs all functions required to insure proper medication management (as defined by The Joint Commission- storage, preparation, dispensing, use and administration) according to accepted standards of medical, nursing and pharmacy practice and applicable hospital policy and/or law. Performs or is able to demonstrate project management skills to extend practice of pharmacy, compliance with pharmacy regulatory function, and engagement in operation of hospital in up to FOUR areas of department/hospital functions (examples would include medication recall management, specific regulation/standard practice/compliance monitoring, drug traceability/security, or clinical practice initiative).

Responsibilities:

  1. Medication order processing and dispensing

� Criteria A: Medication order review and processing - Reviews medication order for completeness and appropriateness. Conducts patient profile reviews prior to dispensing. Detects and resolves basic order entry and prescribing problems. Makes retrospective, basic interventions being conscientious of work quality. Medication order processing and dispensing

� Criteria B: Drug preparation and dispensing - Provides properly and accurately labeled medications demonstrating understanding of all drug preparation techniques, executing dispensing of medications in a timely manner and ensuring product safety and patient safety as paramount. � Criteria C: Documentation - Complies with requirements for Pharmacy Department documentation in all areas in a clear and concise. Medication order processing and dispensing

� Criteria D: Purchasing and inventory control - Obtains drugs in select emergency situations

  1. Provision of clinical and drug information

� Criteria A: Problem solving/decision analysis - Identifies the problem and is aware of possible solutions/options Provision of clinical and drug information Criteria B: Drug information - Retrieves information from available resources and provides general verbal and/or written drug information in accurate and timely manner. Provision of clinical and drug information

� Criteria C: Drug knowledge/ Disease State knowledge - Demonstrates understanding of basic pharmacotherapy of disease states. Provides clinical and drug information

� Criteria D: Collects and interprets data to design, recommend, implement, monitor, and modify patient-specific pharmacotherapy in collaboration with other health care professionals to optimize drug therapy.

  1. Decentralized pharmacist role : patient teaching and communication

� Criteria A: Actively participate in a multi-disciplinary round by identifying medication related contributions to facilitate patient discharge or readmission prevention Decentralized pharmacist role : patient teaching and communication Criteria B: Educates Patient - Counsels all patients about their drug therapy to ensure safe medication use necessary to optimize patient outcomes, and demonstrating respect for patient choice, and demonstrating a pleasant attitude. � Criteria C: discusses patient needs with team of physician and nurses.

  1. Maintains professional competency

� Criteria a: Maintains current drug knowledge and keeps current with trends in pharmacy practice. Maintains professional competency

� Criteria C: able to represent department and profession of pharmacy at committee or task force meetings AND report a professional summary back to department manager and staff Maintains professional competency

  1. Oversee/check/monitor Pharmacy Technician work -per CT pharmacy practice act (regulations)

� Criteria A: aware of and responsible for overseeing all activities pertinent to drug preparation, dispensing and distribution by a pharmacy technicians; conducts in-process and final checks on the performance of all pharmacy technicians. per CT pharmacy practice act/regulations.

� Criteria B: oversees and directs (priorities) daily activities Pharmacy Technicians

  1. Project management skills to extend practice of pharmacy

Demonstrate current knowledge, ability to work and comply with all pharmacy regulations, specifically USP 797 {standards for preparing compounded sterile preparation} and USP 800 {standards for protecting workers while handling hazardous medications/substances}, Joint Commission medication management, State of CT drug laws, and CMS code of participation

Other Information:

Essential:

  • Licensed Pharmacist

  • Bachelor's Degree

Required: Bachelor of Science degree in Pharmacy from an approved college of Pharmacy. New pharmacy school graduate or limited experience, defined as 0-3 years Pharmacy practice experience or has not actively participated in pharmacy practice for > 5 years' registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut. Must obtain a Connecticut pharmacist license within 3 months of employment.

Minimum Experience: zero to six months.

Required: Registered as a licensed pharmacist or eligible for reciprocity in the State of Connecticut. Must become a Connecticut licensed pharmacist within 3 months of employment.

Essential:

  • Bachelor's Level Degree. Licensed Pharmacist. Graduate from ACPE accredited pharmacy program (PharmD program or BS pharmacy).

Demonstrates knowledge, interprets, and complies with all state, federal, and malpractice laws of pharmacy practice. Demonstrates knowledge of USP 797 , USP 800, and use of personal protective equipment Contributes to teamwork with all team members and creating a positive culture.

Computer Skills Required: Able to use computer systems (PC, Microsoft Office: Word, Excel, PowerPoint),

Computer Skills preferred: Preferred (Cerner, Omnicell, MS Access, Simply797, Tracelink).

Desired: Willing to become or is a member of a local, regional, and national professional organization. Participates in departmental and interdepartmental educational, drug use evaluation, quality assurance programs and other projects as required.
