Job Details Job Location Avivo Village - Minneapolis, MN Position Type Full Time Education Level High School Diploma/GED Salary Range $20.53 - $25.67 Hourly Job Shift Day Job Category Housing Support Services Description ABOUT US: Avivo's Ending Homelessness division provides pathways to housing for individuals experiencing homelessness. We work with county coordinated entry systems and tribal governments and operate unique street outreach teams to connect unsheltered individuals with shelter and housing. We assist individuals in finding and maintaining housing, supporting them in scattered-site apartments and site-based housing. We provide transitional recovery housing for individuals and families participating in Avivo's treatment programs. We operate Avivo Village to address the needs of individuals experiencing unsheltered homelessness. Avivo Village is a low barrier and culturally responsive shelter with individual "homes" focused on moving residents to permanent housing. Avivo's Ending Homelessness Division strives for the highest quality of services using culturally responsive approaches and a deep commitment to harm reduction and housing first through trauma-informed service delivery. YOUR ROLE: The Street Outreach Navigator will work with individuals experiencing long-term, chronic unsheltered homelessness and opioid use disorder. You will help individuals obtain housing, refer them to appropriate resources, assist with access to basic needs, support substance use or mental health treatment goals, and establish medical care through progressive engagement. This role is community-based, supporting people in their current environment, which may include streets, under bridges, in parks, camping, and other locations unfit for human habitation. This role thrives in an unpredictable work environment and requires working partially outside year-round. KEY RESPONSIBILITIES INCLUDE: Provide housing-focused street outreach services. Connect individuals to supportive services and housing options. Coordinate with Avivo Supportive Housing programs and other service providers. Complete paperwork accurately and promptly. Respond to voicemails, emails, and other communications promptly. Protect client data according to HIPAA standards. Collaborate with team members, other programs, and external organizations. Develop and maintain collaborative relationships with community partners. Maintain individual and team productivity goals. Attend and participate in required meetings. Qualifications WHAT YOU OFFER: High school diploma required; BA in Behavioral Science preferred or Lived Experience of homelessness and/or SUD/Mental Health recovery. One or more years of experience working with single adults experiencing homelessness or similar experience. Experience with and commitment to working in a diverse, continuum of services in a fast-paced, growing organization. Experience with and commitment to developing and delivering culturally responsive services. PREFERRED: Lived experience of homelessness and/or SUD recovery. Street outreach experience with unsheltered individuals. Work experience in a culturally diverse environment. Experience with programs serving individuals with dual diagnoses. Experience using HMIS and/or Credible. Experience in Harm Reduction and Housing First with knowledge and commitment to these modalities. REQUIERED, KNOWLEDGE, SKILLS AND ABILITIES: Openness to Harm Reduction and Housing First and a willingness to learn. Basic knowledge of mental health, chemical health, community resources, trauma, institutional racism, gender issues, tenant's rights. Ability to provide person-centered care and use person-centered language. Strong relationship-building skills with awareness of healthy boundaries. Strong communication skills (written and verbal). Basic Microsoft Suite knowledge and ability to learn additional internal systems. Knowledge of community resources and ability to proactively advocate. Commitment to staying curr nt in the field and creative problem-solving abilities. Ability to de-escalate and mediate situations. Flexible mindset to deal with various situations. Ability to assess a wide range of client needs.
