We Fairview Health Services are looking for a driven Office/Intake Coordinator to join the hardworking Kidney Acquisition - Deceased Donor Transplant Team on the U of M campus in Minneapolis, MN! This role is approved for 80 hours every two weeks and is benefit eligible. M Health Fairview Transplant Care leads the region with more than 1,500 procedures to date. Our kidney transplant program is one of the largest and oldest in the world. The first kidney transplant in Minnesota was performed at the University of Minnesota, and the world's first kidney-pancreas transplant was performed here in 1966. We pioneered kidney transplants in children and in diabetics. Our nephrology program is consistently ranked by U.S. News & World Report as high performing. To date, we have performed more than 9,000 kidney transplants.

Responsibilities Job Description

As a member of the care team, the Transplant Intake Coordinator supports quality, patient, and family centered care. The Intake Coordinator is responsible for processing and documenting referrals, completing intake calls, scheduling of evaluations, and coordinating and participating in evaluations as needed. The Intake Coordinator will communicate closely with the RN Transplant Coordinator to provide a full continuum of care for the patient.

  • Answers & assists with returning patient calls as advised by coordinator. Fields all new referral calls

  • Facilitates referral to evaluation process in partnership with Transplant Coordinator

  • Requests and sorts medical records needed for transplant evaluation

  • Educates patients on the referral and evaluation process

  • Partners with patient financial representatives and transplant nurse coordinator to ensure financial approval for evaluation

  • Processes prescription refill calls as taught by the coordinator.

  • Works in conjunction with transplant schedulers, transplant administrative staff and coordinator.

  • Assists with preparation of paperwork for transplant candidate and potential living donor patient presentation to committee

  • Prepares letters to referring physicians regarding committee results. Prepares records for insurance companies for financial clearance.

  • Assist in working with schedulers for any other testing

  • Prepares for clinic visits by obtaining results and updating electronic flow sheets

  • Assists with maintenance of patient electronic files. Processes and maintains records sent to storage.

  • Assists with coordination of post-transplant care including scheduling routine visits and testing per post-transplant protocols and communication with patients.

  • Prepares itineraries for post-transplant protocol testing and appointments and sends to patient

Qualifications

Required

  • 2 years clinical experience in outpatient or inpatient environment.

Preferred

  • Experience with high acuity patient populations

  • Graduate of an accredited post-secondary practical nursing program or Bachelors Degree

  • Medical assistant or related healthcare certification

  • LPN
