JOB REQUIREMENTS: JOB SUMMARY: The Community Health Worker (CHW) will serve to connect the community with the health center. This culturally and geographically connected individual will serve as a link between communities, existing community resources, the Gerald L. Ignace Indian Health Center\'s (GLIIHC) Health Promotion Disease Prevention (HPDP) team, and clinical care teams. Through outreach and education, the CHW will assist clients and patients in overcoming barriers to managing and preventing chronic health conditions. In partnership with patients and their medical care team the CHW will refer patients to internal and external self-management programs when appropriate. This CHW position will be a clinic-based community position. Responsibilities may change with Grant Strategies. ESSENTIAL DUTIES AND RESPONSIBILITIES: Assists in fulfilling CDC Good Health and Wellness in Indian Country- Healthy Natives MKE grant requirements, outreach, and activities. Participates as a collaborative team member within the Health Promotion Disease Prevention team of registered dietitians, community health workers and a community outreach coordinator. Provides education to the community and patients of the health center on various health promotion and disease prevention topics including the prevention and management of chronic diseases such as: pre-diabetes, diabetes, heart health, hypertension, cancer screenings and prevention, etc. Provides education, support, follow up, and care coordination to patients to assist with managing and preventing hypertension though facilitation of a self-measured blood pressure program (SMBP). Provides education, support, follow up, and care coordination to patients to assist with reducing tobacco use though facilitation of a smoking cessation program. Provides education, support, follow up, and care coordination to patients to assist with preventing diabetes though facilitation of a CDC diabetes prevention program. As a member of the medical care team, practices within scope to support patients to follow care plan set by primary care provider. As a member of the medical care team, completes timely medical charting and coordinates with other team members to improve patient care and outcomes. Participates in team-based cancer prevention grant work, outreach, education, navigation to screening, referrals, and awareness building events.
