Healthy Communities is CHC’s Public Health AmeriCorps program designed to create cohesion in vulnerable communities and to promote healthcare and support services through outreach, awareness, linkage to care and support. Healthy Communities also serves to identify the unique and integral needs of our communities and neighborhoods and to establish strategic plans to address them. Service delivery will directly address the public health crisis in vulnerable communities and make healthcare and support services accessible through flexible delivery of services. AmeriCorps members will conduct community outreach, education, patient engagement, and link patients to CHCI care and referrals to community based assistance. Additionally, AmeriCorps members will participate in statewide and local activities which promote awareness of health and health services.

This is an AmeriCorps Member role – Community Engagement Specialist – that supports the Community Engagement Department in Middletown.

Terms & Program Benefits:

  • One-year service term from September 3, 2024 thru August 31, 2025

  • Serve a minimum of 32 hours a week for a total of 1700 hours during service year

  • $22,000 stipend over the course of one year, paid bi-weekly

  • An education award of $7,395 at the end of successful service term completion

  • This is not a staff or volunteer position.

As a Community Engagement Specialist for the Community Health Center, Inc., you will:

  • Conduct monthly patient health education workshops; monitor the completion of pre-post surveys by workshop participants; and complete data entry for workshops and surveys.

  • Track and document all completed patient engagement activities in patient’s electronic health record.

  • Serve as the liaison between staff, patients, and community partners related to food insecurity by connecting individuals to the most appropriate entity for assistance relaying information about food insecurity provided in meetings.

  • Establish a food insecurity education and promotion campaign that targets the CHCI patients and under-resourced community members to improve health outcomes by creating patient friendly teaching curriculum/recipes/pamphlets, marketing tools and materials.

  • Collaborate with CT Foodshare on activities and events for the community.

  • Coordinate, conduct, and participate in community outreach and tabling events.

  • Gather and compile a comprehensive resource guide on how to find healthy and affordable foods that can be shared with patients.

  • Create user friendly content/materials for patients on how to access food resources.

  • Conduct community partner presentations to promote CHC’s “Off The Tracks” Program that focuses on awareness of food insecurity.

  • Coordinate and facilitate “Off The Tracks” activities and/or events.

  • Promote and recruit patients to participate in the “Off The Tracks” Program via survey creation and distribution.

  • Connect with patients to gather feedback regarding food insecurity programs and programs and gain new programming ideas via development and implementation of program surveys.

  • Collaborate with CHC dieticians and providers to assist patients in selecting healthy meal options to incorporate as part of a healthy lifestyle.

  • Collaborate with School Based Health staff across the state to build classroom educational sessions about healthy foods and create school gardens.

  • Participate in the planning and networking for the Urban Garden Greenhouse project, to include: o collaborating with Middletown and surrounding area schools for visits, o collaborating with the master gardener on supplies needs and scheduling of events, o connecting with community partners on scheduling and planning events and activities, o serving with the member supervisor to research and collaborate with new community partners, and o collaborating with CHC Communications around messaging for activities and events connected to food insecurity and the green house.

  • Create new initiatives and opportunities around community engagement based on community needs that match the AmeriCorps/CHC mission.

  • Participate in team meetings to provide updates on engagement strategies and success of events, activities and workshops.

Qualifications:

  • High school or equivalent required

  • Proficiency in Microsoft office and internet-related applications

  • Excellent time management and organizational skills

  • Excellent oral and written skills

  • Demonstrated ability to problem solve and remain calm during a crisis

  • Successful clearance of all required criminal history checks (NSCHC)

  • Able to travel between CHC sites and in state

Preferred:

  • Associate’s degree in public health, social work related field

  • Working knowledge of target population and resources available in the community.

  • Experience in coordination of services, advocacy and community outreach in working with vulnerable populations

  • Working knowledge of social determinants of health, specifically food insecurity

  • Experience in project management

  • Have a good understanding of different cultural and ethnic needs regarding food and how that relates to healthcare disparities
