Health First Hospital at Home APP in Melbourne, Florida
The Hospital at Home (HaH) APP is responsible and accountable for providing Quality / No Harm, Customer Experience, and Stewardship. Provides acute, episodic assessment, diagnosis, treatment, consultation, and education as an APRN or PA, trained in a clinical specialty. The HaH APP will collaborate with the interdisciplinary team to deliver hospital level of care to patients in their home envroment, revising plan of care to meet diverse demographics and needs, enhance patient experience, decrease readmissions and improve patient actions.
Primary Accountabilities * Develops a plan of care consistent with established clinical protocols and demonstrates competent skills in therapeutic, cost-effective pharmacologic administration and management. * Perform complete comprehensive assessment at every patient visit with the goal to reduce hospital readmission and/or reduce length of stay * Demonstrate appropriate supervision of patient care activities within the healthcare setting. Deliver effective communitcation with other memebers of the healthcare team and promote effective team functioning. * Demonstrate effective critical thinking and prudent judgment in assessment, diagnosis, planning, interventions, and evaluation of outcomes; adjust plan of care as needed * Provide care to patients and families with sensitivity and respect for their needs, expectations, preferences, age, cultural and individual differences. * Will utilize technology to monitor vital signs and provide virtual communication for patient and family. * Travel to/from Holmes Regional Medical Center and patient’s residence. * Schedule is 7 days on, 7 days off; 7am to 7pm.
- Current Florida APRN or PA license.
- Acute Care or Adult Medicine Hospitalist APP experience.
- Current AHA BLS Provider Certification.
