Job Summary

This position is for The Children’s Center for Communication and Development. The social worker will primarily provide on-site and remote support for the Center families and staff including needs assessments, referrals and recommendations, and consultations for specific issues that impede in the health and overall development of children enrolled in the program. Additionally, the position will provide service coordination through the state's early intervention program, First Steps, for children enrolled in the program with collaboration from the interdisciplinary team of specialists at the Center.

Primary Duties and Responsibilities

  • Performs needs assessments, develops plans and measures outcomes, makes appropriate referrals and recommendations including follow-up for families and staff, meeting social work needs in accordance with the National Association of Social Workers (NASW).

  • Consults with families and the interdisciplinary team of therapists and educators, documenting case updates and participating in weekly clinical meetings to share information.

  • Maintain and update a resource list for appropriate referrals with outside agencies and organizations based on residency of enrolled families and families seeking services on the waitlist.

  • Coordinates referral, evaluation/assessment, Individualized Family Service Plan (IFSP) development and maintenance, provision of services, and transition to school district, meeting all federal indicators for successful provision of services under Individuals with Disabilities Education Act (IDEA) Part C for children ages birth to 3 years.

  • Participate in the waitlist management team, managing the organization and process of identifying high priority cases in an organized manner, reaching out to families under the supervision of the Director of Communications and Development.

  • Performs other duties as assigned.

Minimum Qualifications

Master’s degree from an accredited college or university in Social Work with licensure. Three to five years’ experience in social work or clinical experience with an interdisciplinary pediatric group working closely with families of children with disabilities.

Knowledge, Skills & Abilities

  • Demonstrates the ability to create and maintain information for planning and programmatic needs using software that includes, but is not limited to: Adobe Photoshop, Canva, Microsoft Word, and Microsoft PowerPoint, Microsoft Excel.

  • Ability to interpret and disseminate information from federal and state policies and code for the provision of service coordination and direct services under the IDEA law Part C.

  • Ability to remain empathetic and confidential when dealing with a varied group of individuals and family needs, considering race/ethnicity, culture, socio-economic levels, and other potential causes for health disparities.

Preferred Qualifications

Background within a pediatric interdisciplinary team.
