Overview:

This clinical position as a Behavioral Health Specialist (BHS) focuses on managing mild-to-moderate mental health conditions of pediatric patients receiving primary care at Bronson. The BHS is an integral member of the Collaborative Care team, working with the patient's pediatric primary care provider (PCP), a psychiatric consultant, and other mental health providers when available.

 

The BHS will provide diagnostic assessments, symptom monitoring, and care management for behavioral health, including psychoeducation, patient education tools, brief face-to-face interventions in primary care clinics, and collaboration with primary care clinicians. Using evidence-based Collaborative Care approaches, the BHS will:

  1. Screen patients in the primary care clinic for behavioral health needs.
  2. Engage patients in measurement-based care with standardized outcome measures.
  3. Develop and use a patient registry to monitor patient status and progress.
  4. Review the patient registry routinely with a psychiatric consultant for treatment recommendations and consultation.

     

Qualifications:

  • Master's Degree in Social Work required.
  • Minimum of five (5) years' experience in healthcare, with experience in an outpatient clinical setting focused on behavioral health.
  • Current Social Work license in the State of Michigan required.
  • Valid Driver's License in the State of Michigan required.
  • Proficiency and experience with brief, structured intervention techniques (e.g., Motivational Interviewing, Behavioral Activation).
  • Proficiency in developing care plans, setting patient goals, and using Epic and Microsoft programs (Excel, Word, PowerPoint).

     

Responsibilities:

  • Provide screening, diagnostic assessments, and treatment recommendations for common mental health and substance use disorders.
  • Maintain an overall caseload of 60-80 patients across several primary care practices.
  • Perform co-visits with primary care providers and clinical staff as needed.
  • Engage with patients in practice, via telephone, and via secure patient portal.
  • Create and coordinate patient-centered treatment plans, including physical and behavioral health symptoms and goals.
  • Support psychotropic medication management as recommended by psychiatric consultants and prescribed by PCPs.
  • Systematically monitor patient symptoms, outcomes, and treatment response at regular intervals.
  • Facilitate changes in treatment plans for patients not improving as expected in consultation with the PCP and psychiatric consultant.
  • Develop self-management and relapse prevention plans with patients.
  • Facilitate referrals and coordination with indicated services both internal and external to the practice.
  • Manage clinic-specific patient registry, track patient contacts and outcomes, and re-engage patients as needed.
  • Document patient progress and interventions for the treatment team.
  • Provide patient education regarding mental health and substance use disorders.
  • Use evidence-based techniques such as Motivational Interviewing, Behavioral Activation, and Problem-Solving Therapy.
  • Prepare for and participate in weekly patient panel reviews with consulting psychiatrists.
  • Perform risk assessments, safety planning, and crisis intervention.
  • Participate in additional meetings as appropriate.
