Town of Hebron Adult and Senior Services Social Worker in Hebron, Connecticut
[The Town of Hebron is seeking qualified candidates for the position of Adult and Senior Services Social Worker. This position is responsible for identifying adults, disabled persons 18 years of age and older and senior citizen Hebron residents who may be in need of support services, face-to-face interviewing and assessing clients, family members and/or caregivers, client-centered care plan development, implementation and monitoring, problem-solving, reassessment and quality evaluation. Knowledge of state, federal, and local resources is essential. A Bachelors Degree in Social Work with prior direct case management experience is the minimum required, a Masters Degree is preferred.
