Corewell Health Hospice specializes in providing end of life care and symptom management to individuals with a life expectancy of six month or less, serving individuals in their home, nursing facilities, and in the hospital. About Hospice
In collaboration with the entire interdisciplinary team, assess, plan, implement, coordinate, monitor and evaluate the strategies and services required to meet a patient/family's end-of-life care needs. An outcome-based plan of care is created and continually adjusted through utilization of the nursing process, relationship centered care principles and holistic philosophy assuring the changing needs of the patient/family are met while facilitating a symptom-free, peaceful, sacred death.
Scope of work
Provides spiritual care, counsel, education and spiritual/emotional support to patients, families and staff members of all faiths and traditions. Assists with referrals to faith group leaders when indicated and/or requested. Maintains and applies a working knowledge of medical ethics. Recognizes when ethical principles could or do conflict in actual cases. Recognizes moral distress in care givers. Knows how to initiate an ethics consultation. Able to facilitate advance care planning conversations and decision-making.

Qualifications
Required Bachelor’s degree in ministry, Religion, Theology, or other approved pre-seminary concentration
Preferred Masters Degree in Divinity or Arts in Theology with a concentration in pastoral care
Required one unit of Clinical Pastoral Education (CPE)
Preferred four units of Clinical Pastoral Education (CPE)
Preferred 2 years of relevant experience providing spiritual care in a hospital, agency, or congregation
Required State of Michigan Drivers License
