We seek a Clinical Pharmacist to perform professional duties and responsibilities associated with comprehensive medication management.

You’ll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. This is a telecommuter position.

Primary Responsibilities:

• Assesses medication regimen for indication, effectiveness, safety and compliance

• Provides patient-centered care that will improve clinical outcomes and reduce total cost of care

• Communicates individualized care plans effectively with patients and healthcare providers

• Supports patients in their initiation and maintenance of new therapy; providing education, optimizing utilization and establishing adherence

• Engages patients at clinically relevant times to deliver impactful clinical pharmacy services

• Maintains department performance levels for quality and productivity

• Performs other projects, work, and activities as assigned to ensure continuous service to our patients and clients

• This position may be required to obtain multi-state licensure as necessary for business need

Required Qualifications:

• Bachelor’s degree in Pharmacy or PharmD

• Current unrestricted active pharmacist license in the US

Preferred Qualifications:

• Multi-state pharmacist licensure

• Residency and/or Board certification
