Job Summary :
The Licensed Practical Nurse (LPN) is responsible for providing the initial screening of patients as well as assisting with screenings, procedures, and injections. This position also works with the Registered Nurse to ensure the operational running of the medical clinical area and patient communication within the electronic health record system.
Main Responsibilities :
Clinical Floor Operations
Prepare clinical area for building opening or closing in accordance with Finger Lakes Community Health hours of operation
Redirect nursing team tasks to ensure an efficient patient flow
Clean and sterilize instruments, equipment, and clinical areas according to clinical infection control procedures
Restock rooms with medical supplies
Assess inventory and place order for medical supplies
Perform Quality Control measures including but not limited to:
Monthly control testing
Equipment inspections
Attend morning huddle meeting with patient care team to address patient concerns
Follow-up on referral progress for patients initiated by provider
Review patient information during pre-visit planning to ensure appointments are appropriately scheduled and needed information has been received
Clinical Services
Obtain patient vitals and history during patient rooming process
Notify nurse or provider of abnormal findings during patient rooming process
Assist with minor procedures and exams as outlined in NYS clinical scope of practice
Perform patient screenings as outlined in NYS clinical scope of practice
Complete phlebotomy and lab collection and handling as outlined in NYS clinical scope of practice
Confirm patient charts have written orders for medications and/or injections from the provider
Administer injections and immunizations as directed by ordering provider
Complete medication reconciliation
Review and send medication refill requests to patient’s provider
Inform patients or designated persons of physician orders, changes in care plan, test results, etc. as outlined in NYS clinical scope of practice
Medical Record Maintenance
Scan clinical documents from patient or clinical staff into patient chart
Chart services in patient record including but not limited to:
Referrals, medications, vaccinations, injections, screenings, lab orders/results, patient history, vitals
Chart and lock notes within timely manner in accordance with Finger Lakes Community Health policies and procedures
Address telephone encounters, actions, and referrals in the electronic health record system in accordance with Finger Lakes Community Health policies and procedures
Education and Qualifications:
High School Diploma or equivalent
A current unrestricted NYS LPN license and CPR certification required
Prior experience a plus, but not required
The ability to work flexible hours -- some evenings are required
The ability to communicate fluently in Spanish and English preferred, but not required
A valid New York State driver’s license in compliance with FLCH drivers’ policy
Physical Requirements:
Must be able to hear and communicate with clients and staff via phone, web media, and in person.
Must be able to lift up to 40 lbs
Must be able to sit for extended periods of time
Must be able to kneel, crawl, or bend into awkward spaces
