What You'll Do

 

NOTE:* Please know this position will need to work in Everett and Renton (few days of the week)*

 

The SSVF Healthcare Navigator / Case manager will work closely with Homeless and Formerly Homeless Veterans and their Families and is part of the YWCA's Specialized and Integrated Services Division. The SSVF Healthcare Navigator works closely with Veterans to assist them in communicating their preferences in care and personal health-related goals to facilitate shared decision making of the Veteran's care. The SSVF Health Care Navigator serves as a resource for education and support for Veterans and families and helps identify appropriate and credible resources and support tailored to the needs and desires of the Veteran.

 

In addition to healthcare navigation services, SSVF Healthcare Navigator / Case managers will identify and engage with current and previously homeless families to assist them in obtaining and retaining permanent affordable housing. This will require establishing effective case management relationships with formerly homeless veterans and homeless veteran families, building relationships with property managers and other social service providers in coordination with the SSVF Program Manager I.

 

This position assists in connecting families to supportive services and healthcare services to help ensure success in permanent housing. This position requires the SSVF Healthcare Navigator/Case Manager to conduct assessments on all veterans enrolled within the program. This requires working with the SSVF team to ensure that they are part of the enrollment process for all clients being brought into the SSVF program. In addition, this position will be expected to respond to crisis situations which could result in loss of permanent housing if not resolved. This position has a caseload of families seeking housing as well as families experiencing difficulty in maintaining housing. 

Responsibilities

Conduct Healthcare assessments and determine program eligibility upon enrollment into the SSVF program.

Understand the VA and outside medical healthcare systems.

Be knowledgeable and build relationships within the medical, mental, rehabilitation, and treatment centers.

Ensure veterans are reminded of their healthcare appointments.

Provide referral and connection to supportive services, such as chemical dependency treatment, mental health counseling, domestic violence advocacy, etc. 

Provide outreach to locate, identify, engage and refer homeless and formerly homeless veterans and their families to benefits and services.

Provide links to housing and housing stability resources. 

Build relationships with landlords/property managers. 

Work in collaboration with other social service providers in the community. 

Assess the strengths and challenges of the client and assist in developing housing stability goals, individual goal plans, and action steps.
