Community Health & Resource Navigator
Full-time position
Healthy Acadia, a non-profit community health coalition providing health services primarily in
Washington and Hancock counties and as well as across Maine, seeks a Community Health &
Resource Navigator to join a dynamic team passionate about making it easier for all people to
lead healthier lives. Reporting to the Recovery Projects Director, the Community Health &
Resource Navigator will support care navigation through the HRSA PROSPER Initiative and
through the HRSA RCORP work, serving individuals with substance use disorder, their children
from birth to age three, and affected others. The position will serve Hancock County, Maine.
There will be travel throughout the service area to support clients, in addition to offering virtual
support.
Responsibilities:
? Provide direct support for community members with substance use disorder, their
children, and affected others to find, access, and navigate treatment and recovery
supports; will also help them to meet their unmet needs like food assistance, employment,
and medical care
? Serve as an expert link between individuals and these community resources and
collaborate with other navigators, case managers, medical professionals, recovery
coaches, and recovery center staff to provide seamless navigation support
? Work with the PROSPER team and other partners to provide education and outreach
about PROSPER
? Provide technical assistance and expertise on availability of resources for service
providers, individuals, families, and caregivers
? Work in partnership with recovery coaches to receive and make referrals
? Serve as a liaison in Hancock County hospitals and other healthcare settings to assist with
supportive discharge to recovery services
? Help determine recovery preferences of community members, families, caregivers, and
support the Activities Coordinator in the creation and implementation of recovery
activities and groups
? Attend quarterly PROSPER consortium meetings
? Attend Hancock County Community Advisory Board meetings
? Attend the monthly Downeast Substance Treatment Network meetings
Qualifications and Experience Sought:
? Working knowledge of local and regional health care systems, social services, and other
resources preferred
? Personal experience with the recovery process and/or knowledge of pathways to recovery
? Experience working with vulnerable populations
? Team player able to elevate the success of colleagues and community

? Ability to manage and disseminate information effectively to a wide variety of audiences
? Strong communication skills: ability to listen well, write and speak effectively
? Highly self-motivated, ability to work independently from the office or in the community
? highly organized with the ability to work on multiple projects concurrently, set and keep
deadlines, regularly prioritize work and follow own plan of work
? Warm, personable, and approachable
? Ability to work with diverse people, conduct culturally competent outreach, and build
and maintain strong working relationships with diverse partner organizations
? Ability to exercise good judgment in the face of competing priorities, agendas, and
partner sensitivities
? Bachelor\'s degree in Social Work or similar field, or equivalent experience
This position is full-time (40 hours/week) with strong benefits. Salary range: \$19-\$21/hour. We
are seeking a full-time person, but would consider part-time as well. Regular travel throughout
the project service area and occasional travel beyond is required.
