Henry Ford Health System Supervisor Case Management in Detroit, Michigan

GENERAL SUMMARY:

Under the direction of the Manager of Case Management, supervises functions of case management including basic discharge planning activities to achieve desired outcomes. Participates in the establishment of policies, procedures and work processes. Maintains performance improvement activities within the department and participates in the department services first goals and objectives.

EDUCATION/EXPERIENCE REQUIRED:

Masters of Social Work required. Minimum of three (3) years clinical experience; hospital-based case management preferred. Working knowledge of Case Management. Communication and interpersonal skills. Analytical, critical thinking and problem-solving skills. Outcome measurement focused. Computer skills, including software such as Word, Word Prefect, Excel, Power Point, Visio/Flow charting. Positive physician and payer relationship.

CERTIFICATIONS/LICENSURES REQUIRED:

Full License (LMSW) to practice as a Social Worker in the State of Michigan. Certification in Case Management (CCM) by the Commission for Case Management Certification (CCMC) preferred. For current employees, effective date 10/31/2019, certification is required by December 2023. For new hires, certification must be obtained within 3-years from date of hire.
