Service Source Program Manager - Rehabilitation, Community Integration in Clearfield, Utah

Job Summary

Oversee the daily operations of the Community Integrated Services program. This includes oversight of all participants, staff budgets, communication between all funding source representatives, parents, caregivers and guardians. This position also requires leading out on all identified outcomes from leadership and stakeholders for the assigned program.

Primary Duties

  • Supervise assigned Rehabilitation personnel to include hiring, training and completing performance evaluations. Ensure their duties and responsibilities are completed accurately, timely, and in compliance with regulatory bodies and funding source requirements.

  • Manage rehabilitation support programs to ensure compliance and contracts are adequately staffed.

  • Ensures appropriate and comprehensive services provided to persons receiving services are in conjunction with contract specifications and supervisory instructions.

  • Stay abreast of Division of Services for People with Disabilities, Medicaid Waiver, CARF and other appropriate regulatory requirements for compliance ensuring that all required cases records are being maintained.

  • Manage departmental budget to meet goals and financial parameters set by leadership.

Additional Responsibilities

  • Ensure that documentation is completed in an accurate and timely manner including evaluation/assessments, progress reports, score cards, case notes, incident reports, etc. In addition, ensure the case management system is up to date and accurate.

  • Meet with representatives of other community agencies, families, and significant others for the purpose of employee program planning and review.

  • Assist in negotiation of fees for service and contracts with local, state and federal funding sources.

  • Works with local businesses within the community to secure employment and contract opportunities for participants.

  • Coordinate and participate in the interdisciplinary team process and other meetings pertaining to individual’s goals and objectives as needed.

  • Act as liaison with state and local rehabilitation agencies and case management services.

  • Coordinate services with funding sources, local businesses and other stakeholders to facilitate program services and participant employment opportunities.

  • Support Rehabilitation Services objectives as outlined in the Business Plan and Program Evaluation Plan.

  • Maintain a working knowledge of the Ticket to Work program.

  • Perform other responsibilities as assigned.

Qualifications: Education, Experience, and Certification(s)

  • Bachelor's degree in Special Education, Vocational Rehabilitation, Social Work or related field, plus 4 years’ experience working in a rehabilitation program required. In lieu of education, 8 years of progressive experience working in a Rehabilitation program may be substituted.

  • Four years previous supervisory experience is required.

  • Prior experience working with individuals with disabilities is required.

  • Valid driver’s license and/or access to reliable transportation to perform work-related travel required. Eligible drivers must have a good motor vehicle record (MVR).

  • Pass and maintain Department of Human Services background clearance required.

Knowledge, Skills, and Abilities

  • Working knowledge of Utah community support agencies and funding issues for training and support of individuals with disabilities.

  • Demonstrated experience in supervision and management of professional and rehabilitation staff.

  • Demonstrated experience in development and management of budgets, forecasts, and financial operations.

  • Ability to read, analyze, interpret, and prepare complex operating documents.

  • Ability to work effectively as a supervisor, peer, and subordinate to accomplish necessary work.

  • Effective internal and external customer relations skills; ability to participate in and respond effectively to sensitive inquiries, negotiations, complaints, and team activities

  • Ability to make effective speeches or presentations on a variety of topics to stakeholder groups.

  • Experience with Microsoft Office Suite and the ability to learn corporate specific programs.

  • Ability to respond effectively to the most sensitive inquiries or complaints.

Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and ability to adjust focus.
