We’ve partnered with a facility and are currently seekingan Optometrist for an opening in Chicago, IL. This is a 12-month contract.

DETAILS & RESPONSIBILITIES:

• Competitive Pay

• M-F (8am to 4pm)

• Start Date – ASAP

• Credentialing Estimate – 1 month

RESPONSIBILITIES SUMMARY

• Full-scope primary eye care.

• Obtaining and documenting in the EHR an ophthalmic andmedical history.

• Comprehensive ophthalmic exams, accurate and completediagnoses, and effective treatment plans

• Treatment plans will include (as clinically indicated)the prescription of eyewear, contact lenses, oral and ophthalmic medications,and ordering ancillary testing as needed.

REQUIRED QUALIFICATIONS

• Doctor of Optometry (OD)

• Active/Unrestricted Optometrist Licensure in IL

• 2+ years of clinical experience
