Job Summary

This role supports Atrium Health and is responsible for providing Health and Wellness services for underserved teammates in the surrounding communities. The services will be based off submitted caseloads. This position will assist teammates by sharing and locating resources for housing, food, legal services and mental health care, while ensuring they’re in an environment where they feel safe.

ESSENTIAL TASKS:

  • Utilize Atriums HELP NOW platform to identify teammates in need of support and resources.

  • Act as a partner and ally for Atrium Health Teammates to create housing stability.

  • Liaison between teammate and the emergency care fund division to assist in getting financial approval.

  • Assist individuals in underserved communities in completing benefit applications.

  • Build and maintain relationships with community partners to best be able to assist teammates with needed resources.

  • Communicate with teammates department managers to ensure proper support for teammate.

  • Educate families on the importance of becoming healthy mentally, financially, and physically.

  • Conduct follow up meetings with teammates 30/60/90/1 year later to ensure continued stability, and to track program successes and opportunities.

  • Provide continuing education to teammates who have received assistance to ensure upward mobility.

Education

Bachelor’s degree required, preferably in Health Education. MWS Preferred.

Experience

Two years’ experience in healthcare required. Ability to communicate (both verbal and written) effectively. Ability to apply elementary math functions. Ability to apply common sense understanding to carry out detailed, but uninvolved written or oral instructions. Ability to work well under time constraints.
