Outreach and Engagement Specialist (CV2621-219) - Safe Return Division

Volunteers of America Delaware Valley is looking to hire an Outreach and Engagement Specialist who shall report to the Regional Director.

The responsibilities include:

  • The coordination of care management: providing information to vulnerable populations in various settings throughout the community;

  • Assessing the emergency needs for each individual requesting service;

  • Provide immediate, direct services

  • Working as part of the team for effective care management.

  • Provide highly individualized services in the community and promote client self-determination and decision-making.

  • Provide essential expertise and consultation to the team to promote a culture in which each individual's preferences are recognized and integrated into treatment, rehabilitation, and community self-help activities.

Scope of Duties: 1. Interact (verbally and non-verbally) with all clients, staff, and community members professionally with respect and courtesy. 2. Maintain client and staff confidentiality in compliance with administrative policies and procedures. 3. Attend and participate in regularly scheduled staff meetings, supervisory conferences, and training sessions and meetings as required. 4. Participate in regular street outreach activities and community events to engage individuals within the community. 5. Disseminate information regarding services provided by the agency to the community. 6. Responsible for the coordination of transportation for individuals requesting services. 7. Coordinate comprehensive intake assessment procedures on all engagements. 8. Communicates special needs and represents client's interests to program staff. 9. Network with other community agencies to develop resources for clients. 10. Complete Service Plans on all assigned clients based on approved agency assessment tools with client input. 11. Monitor and document client progress according to funding source standards and/or operational requirements in real time. 12. Fulfill obligations of training agreements entered into for core curriculum programs. 13. Develop appropriate discharge plan on all assigned clients as per contractual requirements and policy and procedures. 14. Complete necessary reports and case-related paperwork in a timely fashion. 15. Facilitate regular group sessions for clients as directed. 16. Provide practical assistance for clients when they first enter the facility by mentoring, advocacy, coordination, side-by-side individualized support, problem-solving and direct assistance to help clients obtain the necessities of daily living including utilizing nutritional, financial, physical, and mental and emotional resources. 17. Assist in maintaining communication between all staff members and clients. 18. Complete other duties as assigned.

ADA Essential Functions: 1. Regularly required standing, walking, sitting, using hands to handle or feel, reaching with hands and arms, climbing stairs, talking, hear, and smelling. 2. Regularly lift and move up to 25 lbs. 3. Ability to see clearly at 20 inches or less. 4. Ability to identify and/or distinguish colors. 5. Ability to observe an area that can be seen up and down or to the left and right while remaining fixed on a given point.

Credentials/Abilities:

  • A High school diploma or equivalent with a minimum of two year’s work experience in a relevant field.

  • Direct experience in working with the homeless population and individuals who suffer from substance abuse and mental health issues.

  • Possess effective written and verbal communication skills and an ability to interact appropriately with clients.

  • Must be able to pass a criminal background check.

  • A valider’s license is required and the ability to meet agency driving standards.
