Position Summary:

The Risk Manager and Patient Safety Specialist manages the review, investigation and reporting of safety events/incidents/occurrences with potential impact on patient safety and hospital liability in accordance with established hospital, regulatory, and accreditation procedures, guidelines and criteria. This individual will collaborate with Lahey Clinic colleagues to formulate analyses, and appropriate responses and follow up actions. This individual will be responsible to maintain coding, tracking and event data in the electronic safety reporting system, prepare periodic reports and analyses and presents to internal and external audiences. The individual will be required to provide professional support and education for Lahey Clinic wide risk management and patient safety related activities, and manage regulatory reporting and facilitates regulatory investigations.

Essential Duties & Responsibilities including but not limited to:

1) Responsible for participating in the PCA, Peer and Process Review, and Professional Affairs Committees under the direction of

the Director of Risk Management and Patient Safety and the applicable Medical Chair.

2) Responsible for coordinating the clinical and systems investigation, analyses, of safety events, occurrences, and quality

complaints that are further reviewed through these committees. Conducts interviews of clinical and non clinical colleagues

involved in events; accesses clinical information via medical record and on line information systems.

3) Responsible for coordinating the Root Cause Analysis process for high level safety events as deemed appropriate by the Vice

President of Quality and Safety and the Director of Risk Management and Patient Safety.

4) Maintains safety reporting system, tracking and evaluating safety events/occurrence data. Triages reports, verifies and corrects

data entry of information in safety reports and prepares periodic reports for clinical leaders and regulatory agencies according to

established criteria.

5) In conjunction with Director of Risk Management and Patient Safety maintains communication and collaboration with LCMC

General Counsel, LCMC colleagues, and Claims Department in order to investigate and respond to events/occurrences, advise

on corrective and preventive measures and educate on risk prevention and management strategies.

6) Participates in on-site visits from accreditation and regulatory agencies and follows through on corrective action plans as

identified by regulatory investigation.

7) Provides consultation and education for LCMC colleagues. Advises on risk management and patient safety issues, requirements

and strategies and works with departments to evaluate and improve internal processes and procedures.

8) Responsible for assisting physicians and colleagues in a proactive reaction to negative outcomes by instructing in

communication style, customer service responsiveness; and by managing proactive communication plans with patients and their

families during the investigation of unexpected outcomes and complaints.

9) Maintains knowledge of current professional industry, legislative, insurance, and regulatory trends and practices through

participation in relevant professional organizations and attendance at conferences and other relevant continuing education

programs.

10) Works as a dynamic member of the Risk Management and Patient Safety team, sharing knowledge, expertise, and workload.

Collaborates with members of the Quality Improvement and Customer Relationship Management Department and other

members of SSMT to optimize organizational systems support to LCMC colleagues.

11) Incorporates Lahey Clinic Guiding Principles, Mission Statement and Goals into daily activities.

12) Complies with all Lahey Clinic Policies.

13) Complies with behavioral expectations of the department and Lahey Clinic.

14) Maintains courteous and effective interactions with colleagues and patients.

16) Demonstrates a commitment toward meeting and exceeding the needs of our customers and consistently adheres to Customer

17) Participates in departmental and/or interdepartmental quality improvement activities.

18) Participates in and successfully completes Mandatory Education.

19) Performs all other duties as needed or directed to meet the needs of the department.

Minimum Qualifications:

Education:

· Advanced knowledge of clinical practices typically acquired by attaining a Master’s degree in nursing or related field

Licensure, Certification, Registration:

· None required

Skills, Knowledge & Abilities:

· Ability to analyze complex clinical and organizational events.

· Knowledge of regulatory requirements, experience facilitating regulatory investigations, and passion for working in a High Reliability Organization.

· Strong knowledge of Risk Management principles typically acquired by ASHRM or CPHRM certification

· Demonstrated knowledge and skills in Quality Improvement and Patient Safety.

· The ability to effectively communicate, collaborate and work through complex and sensitive situations with individuals and groups.

· Analytic individual with strong computer skills

· Ability to utilize common business and clinical software including but not limited to data bases and spread sheets

· Ability to function effectively and efficiently in a stressful and dynamic work environment.

· High level skills in oral and written communication.

· Experience:

· Clinician with at least 10 years in clinical practice which includes prior experience in a clinical practice leadership role.
