Job Description:

To assess the psychosocial needs of patients/families/significant others and provide services in accordance with professional social work standards and in collaboration with the interdisciplinary health care team.

Minimum or Preferred Qualifications:

Bachelor of Science Social Work or related field required. 2 years of experience in counseling preferred. Youth Link only - 2 years of experience providing case management, care coordination, and supportive services to individuals at risk or living with HIV in an outpatient clinic. Youth Link only - 2 years of experience providing case management, care coordination, and supportive services to LGBTQIA+ individuals in an outpatient clinic.

Salary Range:

$28.86-$36.57 per hour Wage will be determined based on factors such as candidate's experience, qualifications, internal equity, and any applicable collective bargaining agreement.
