This position will support the Risk Reduction section with chronic disease prevention programming through a health equity lens with a focus on early intervention, nutrition, and physical activity. The incumbent will be responsible for contract and subgrant monitoring and reporting, contract and grant application coordination, and award management for the comprehensive cancer, diabetes prevention and, Fit and Fall Proof/Idaho physical activity and nutrition programs.

Typical Duties:

  • Plan, develop, implement, coordinate, and evaluate activities carried out under program subgrants. Activities include, but are not limited to:

    • Providing free health screenings to assess individuals risk of diabetes and heart disease.
  • Teaching a cohort style diabetes prevention class.

  • Developing nutrition, diabetes, and physical activity related education materials.

  • Facilitating nutrition focused cooking classes for families, adults, and teens.

  • Become a trained Fit & Fall Proof Instructor and assist with program delivery.

  • Assist in the development of program priorities, policies, and procedures in accordance with program, partner, community, and department needs.

  • Assist in the development of data collection and analysis strategies for evaluation activities.

  • Assist in the preparation of progress reports for federal funders.

  • Develop contracts to reflect federal program requirements, including requirements for training, model fidelity, evaluation, data systems, consultation, and continuous quality improvement.

  • Monitor contracts to ensure that authorized activities, responsibilities, and deliverables are met and conduct site reviews as needed to evaluate compliance with state and federal requirements.

  • Identify community needs related to risk reduction efforts.

  • Coordinate and disseminate consultation, training, and technical assistance for community partners and partner organizations.

  • Maintain and develop strategic partnerships with key stakeholders and community organizations.

  • Assess program budget needs and prepare and justify annual budget request in coordination with the program manager.

  • Develop and present information to varying audiences.

  • Where appropriate, assist with the coordination of program plans with other programs and divisions within CDH.

    Minimum Qualifications:

  • Good knowledge of:

    • Public health education principles and practices to include needs assessment, information dissemination and community organization relating to common health problems.
  • Basic disease etiologies and preventive strategies.

  • Survey methods and basic statistics used to collect and interpret data.

  • Training methods.

  • Experience developing or reviewing and evaluating health-related informational material.

    Preferred Qualifications (these are not required, however, having these qualifications may increase your overall ranking):

  • Ability to speak, write and read both English and Spanish.

  • Experience working with the Hispanic/LatinX population
