Covenant Health DIRECTOR POPULATION HEALTH & CLINICAL OPERATIONS in Bangor, Maine

Job Summary:

The Director of Population Health and Clinical Operations at Community Care Partnership of Maine will support the clinical operations and quality teams to lead and implement the organization’s clinical operations and population health strategy as well as develop, synthesize, disseminate and implement population health best practices for the organization. This position serves as the primary clinical quality point of contact for CCPM’s member organizations . The ideal candidate will have experience using a population health tool (e.g. Azara, Arcadia) and experience successfully planning, organizing, managing and implementing care management and quality projects across an organization.

  • Key Responsibilities:

  • Work hand-in-hand with the COO, CCOO and CMO to lead and implement the organization’s clinical operations and population health strategy.

  • Develop, synthesize, disseminate and implement population health best practices for the organization and serves as the primary clinical quality point of contact for CCPM’s member organizations.

  • Act as a subject matter expert and resource to member organizations working to implement and improve upon existing population health strategies and clinical operations workflows

  • Oversee and support the interpretation, dissemination and utilization of actionable data across CCPM member organizations in order to advance population health

  • Support the formation, development and ongoing monitoring of members organizations’ population health and quality programs.

  • Conduct onsite visits or otherwise connect with member organizations as needed to support training and deployment of population health strategies and clinical processes.

  • In collaboration with COO, CMO, and CCOO, support the development of grant applications and reporting requirements related to funded projects.

  • Attending and actively engaging at the quarterly member organizations collaborative meetings, acting as a subject matter expert and resource in population health initiatives and clinical operations, ensuring forward movement in various strategic initiatives.

  • Participate in payer meetings to review performance, provide updates regarding CCPM’s population health strategies and clinical operation initiatives/efforts and facilitate performance improvement efforts with individual member organizations as needed.

  • Oversee the completion of payor specific performance improvement plans, reporting and other deliverables.

  • Oversee annual GPRO, Medicare clinical quality measures (Medicare CQMs), MaineCare and other payer reconciliation audits.

  • Drive the performance of CCPM though patient-centered care management initiatives

  • Oversee and coordinate education and workflow development to support the effective use of the health information exchange, population health tool and other resources in collaboration with key stakeholders.

  • Support identification, standardization and implementation of an evidence-based care management curriculum across CCPM member organizations

  • Lead the Care Management Subcommittee meeting

  • Support Chair in preparing for and leading the Quality and Clinical Integration Committee meeting.

  • Perform other duties and/or responsibilities as assigned.

  • Education & Experience:

  • A minimum of 5 years in healthcare quality and/or care management/population health management role.

  • Experience using a population health tool (e.g., Azara, Arcadia) required.

  • A bachelor’s degree in nursing or a related degree required.

  • Demonstrated experience successfully planning, organizing, managing and implementing care management and quality projects across an organization

  • Experience with government and payer programs and their regulations, guidelines, and standards strongly preferred.

  • Experience with or exposure to Accountable Care Organization Operations strongly preferred.

  • Exceptional leadership, strategic and day-to-day project and task management skills.

  • In-depth industry knowledge of ACA, MACRA, MSSP, Population Health, Clinical Integration, and Value-Based Payment Models strongly preferred.

  • Excellent written and verbal communication skills.
