Be Finally Free, Inc. Licensed Clinical Social Worker (LCSW) in Bakersfield, California

Job Summary: Be Finally Free (BFF) is seeking a dedicated Licensed Clinical Social Worker (LCSW) to oversee the Enhanced Care Management (ECM) program, which aims to provide long-term support and coordination of services for high-risk, high-cost Medi-Cal managed care members with complex medical and social needs. The LCSW will ensure the program aligns with BFF's mission and goals and will work collaboratively with the ECM team to provide comprehensive care and support to program participants.

Key Responsibilities:

  • Develop and implement the ECM program in accordance with CalAIM guidelines along with adhering to BFF's mission and goals.
  • Provide clinical oversight and guidance to the ECM team, including case managers and support staff.
  • Conduct assessments and develop care plans for program participants, ensuring their needs are met and services are coordinated effectively.
  • Oversee and monitor care plans to ensure they are implemented correctly and achieve desired outcomes.
  • Monitor implementation of Care Plans; recommend revisions or updates as necessary to accomplish the clients goals.
  • Monitor quality assurance, by ongoing review of electronic records and client charts.
  • Monitor caseloads to ensure client safety and program compliance.
  • Collaborate with external partners, including Medi-Cal managed care health plans and community organizations, to enhance program effectiveness and participant outcomes.
  • Monitor program performance and outcomes to make recommendations for program improvements.
  • Ensure compliance with all relevant laws, regulations, and ethical standards.

Qualifications:

  • Master's degree in Social Work from an accredited institution.
  • Current LCSW license in California.
  • Minimum of 2 years of experience in social work, with a focus on care management and/or case management.
  • Experience working with diverse populations, including individuals with complex medical and social needs.
  • Strong leadership and communication skills.
  • Ability to work collaboratively in a team environment.
  • Knowledge of Medi-Cal managed care programs and regulations preferred.

Other Requirements:

  • Ability to travel within Kern County as needed.
  • Valid driver's license and access to a reliable vehicle.
  • Bilingual Preferred.
