Description:

Position Purpose: Mercy Care Atlanta is seeking a dedicated Case Management Coordinator to lead and supervise our (PATH) community support team. This clinical mental health/substance abuse (MH/SA) role involves overseeing the implementation of MH/SA services plans, directly providing MH/SA services, and authorizing individual recovery plans. This position also bridges communication between stakeholders, manages community resources, and supports residents in our care.

What You Will Do:

  • Lead and supervise a community support team.

  • Provide direct MH/SA services and authorize recovery plans for consumers.

  • Collaborate with community service organizations to utilize resources effectively.

  • Conduct street outreach and lead the team in these activities.

  • Supervise and train BA-level interns, including grading.

  • Manage assigned budgets and report program activities accurately and timely.

  • Participate in health fairs or community events 3-4 times a year, including some weekends.

  • Provide patient care and support activities primarily for adults (18-65) and geriatric patients (65+), with occasional care for infants, children, and adolescents.

Minimum Qualifications:

  • Bachelor’s degree in human services or a related field.

  • Preferred licensure: LMSW, LPC.

  • At least three years of experience in MH/SA recovery services, including one year in an administrative/supervisory role and one year working with disadvantaged or special needs populations.

  • Experience with both mental illness and substance abuse/dependence.
