POSITION PURPOSE

Maintains good rapport and cooperative relationships. Approaches conflict in a constructive manner. Helps to identify problems, offer solutions, and participate in their resolution. Maintains the confidentiality of information acquired pertaining to patient, physicians, colleagues, and visitors to Trinity Health. Discusses patient and hospital information only among appropriate personnel in appropriately private places. Behaves in accordance with the Code of Conduct, Service Excellence Standards, and the Mission, Vision, Values and policies of Trinity Health. Assumes responsibility for performance of job duties in the safest possible manner, to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management. Completes screenings and assessments as well as review of clients' electronic health record (EHR) and other pertinent documents. Selects and completes appropriate therapeutic interventions based upon clinical knowledge base, critical thinking, and educational background to formulate an appropriate behavioral care plan in conjunction with the CL Provider and the medical team. Develops goals and interventions in accordance with evidence-based practice and patient agreement. Provides short-term psychotherapy and/or counseling for individuals, families, as needed. Supports a recovery-focused environment and patient-centered treatment approach. Provides patients with self-management skills and educational information so they can be full participants in their own treatment and recovery. Assures complete, accurate, and timely documentation of clinical information in the EHR, incorporating current documentation and billing guidelines and requirements. Assures the consistent delivery of appropriate evidence-based services to achieve measurable improvements in quality of care. Provides behavioral health education and utilizes community resource opportunities for the individual and/or family. Monitors and coordinates the delivery of health services including linking with other treatment providers and community services as appropriate. Maintains a process for systematic follow-up by determining the effectiveness of the treatment plan with documentation of patient care outcomes. Reassesses and modifies the plan in collaboration with the CL Providers, medical team and with the patient as necessary to achieve treatment goals. Maintains strong, working relationships with other members of the treatment team. Provides timely feedback to the CL Providers and other treatment team members regarding patients' care, treatment recommendations, and progress through verbal communication, EHR documentation, systematic comprehensive case review, record review, and informal discussions. Engages in regular communication to jointly assess, prioritize, and respond to patients' care needs. Provides consultation and training to Trinity Health Ann Arbor/Livingston providers and staff.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES

In addition to the Pharmacy Technician Specialist responsibilities, the Home Infusion Pharmacy Technician Specialist is also responsible for:

Prepares IV admixtures for home infusion patients

Interprets and follows complicated compounding formulas.

Maintains knowledge of all equipment used in clean room and policy and procedures associated with them.

Ensures appropriate NDC products are used for compounding for accurate charge capture.

Maintains advanced knowledge of IV stability and storage issues.

Maintains advanced knowledge of prescription labeling information.

Prepares Hazardous Drugs for home infusion patients

Maintains competencies related to hazardous drugs handling and preparation.

Interprets and follows complicated directions for mixing hazardous drugs.

Patient Record Maintenance

Responsible for filing paper work such as HIPPA forms, record release forms, signed delivery tickets in patient charts to maintain compliance.

Follows up with appropriate personnel for missing documents.

Responsible for filing prescription records and compounding records in patient charts.

Organizes current patient charts

Cleans and coordinates storage for inactive patient charts to meet regulatory guidelines for storage of records.

Assist with third party audits by requesting closed charts from the storage.

Shipping and Distribution Assistance

Verifies and confirms shipping/delivery address and times with customers.

Picks supply items for IV infusions at home and prepare for packaging.

Coordinates delivery schedule with vendors for home infusion meds to be delivered to patients’ homes.

Completes final packaging for shipment/delivery once all drugs/supplies are checked by a pharmacist.

Hands off packages to drivers for a delivery to patients’ homes.

Drug Card Billing Assistance

Drug card billing for home infusion pharmacy patients, troubleshoot claims rejections, finalize and adjust drug claims for submission.

Process test claims for patients to provide copay information for patients before they come on service.

Process any required third party billing claims in Towers Pharmacy program and coordinate with home infusion pharmacy program.

Assist with drug ordering and inventory functions as needed.

Complexity of Work: Ability to perform algebraic calculations necessary for the IV admixtures and hazardous drugs preparation.

OTHER FUNCTIONS AND RESPONSIBILITIES

Performs other duties as assigned.

REQUIRED EDUCATION, EXPERIENCE AND CERTIFICATION/LICENSURE

Education: Minimum of six months specialized training beyond High School Diploma

Experience: Minimum 1 year experience in sterile compounding, previous home infusion pharmacy experience preferred.

Licensure: Current State of Michigan Pharmacy Technician license required.

REQUIRED SKILLS AND ABILITIES

Interpersonal skills necessary to communicate effectively within the Pharmacy department, delivery vendors, and with SJMHS personnel. Pharmaceutical knowledge commensurate with a certified pharmacy technician to function on-line as work volume dictates. Ability to concentrate and pay close attention to detail for up to 100% of the time when performing on-line functions. Ability to maintain intravenous aseptic technique standards. Advanced computer skills to perform in an office environment, retrieve data, generate reports, and function in an on-line position. Ability to function in a stressful and demanding environment with critical time constraints.

Physical Requirements

Repetitive use of hands and fingers (e.g. preparing IV admixtures, use of computer keyboard). May require lifting and carrying loads up to 30 lbs., including boxes, and IV solutions and stooping or kneeling (e.g. to pick up items from the floor, to remove and replace items on lower shelves, and to file documents.) Sitting, walking, or standing for long periods of time (4-8 hours) often.
