Provides case management services in accordance with the standards set forth by the Department of Corrections. Further, monitors and ensures offender compliance with the rules of Northeast Indiana Community Corrections and any orders of the court. Essential Functions/Responsibilities: 1. Administers the Indiana Risk Assessment System (IRAS) and any other required assessments per policy 2. Conducts regular case management meetings as it pertains to offender risk level 3. Works with offenders to collaboratively develop a case plan 4. Redirect anti-social attitudes/beliefs and affirm pro-social attitudes and beliefs 5. Prepares and updates offender case plans during offender case management contacts 6. Maintains detailed case notes with regard to case planning and management 7. Maintains accurate case information with regard to offenders supervision 8. Participates in an on call rotation and answers calls 24hours a day 9. Assists with and testifies in court proceedings as needed 10. Prepares violations, status/conduct reports and various other reports as needed 11. Completes discharge paperwork for offenders with probation supervision 12. Provides and/or refers offenders to programming services 13. Facilitates Cognitive Behavioral Groups 14. Collaborates with mental health and substance abuse referral agencies 15. Assists in preparing statistical and other reports as required 16. Effectively communicates any messages of importance to the appropriate staff 17. Responsible for approving and documenting an offenders weekly schedule 18. Assists in monitoring GPS movements and employment verifications 19. Attends weekly staffing 20. Other duties as assigned by immediate supervisor or Executive Director
