Field Mid Level Providers (NP or PA)

Overview:

Seeking compassionate and resilient Nurse Practitioners (NP) or Physician Assistants (PA) to serve hard-to-reach areas in Syracuse. As a Field Mid Level Provider, you will blend into the surroundings, provide medical care in homes, and have the flexibility to work both in the field and virtually.

Primary Responsibilities:

Conduct in-home physical exams and consultations, focusing on preventive care and education.

Ensure patient safety and well-being as the top priority in all circumstances.

Maintain proper documentation and paperwork, utilizing the EMR system MedNT.

Collaborate with the clinical team and act medically smart in emergency situations, handing off cases when necessary.

Provide supplemental and supportive medical care, emphasizing general medicine and patient education.

Qualifications:

3-5 years of experience as an NP or PA.

Active NPI, Medicaid/Medicare, and NP License numbers.

Ability to practice general medicine and educate patients effectively.

Strong communication skills and the ability to blend into diverse communities.
